AFFIDAVIT OF EXPLANATION

I, ELLAINE MAE R. MAHUSAY, of legal age, married, Filipino Citizen,

and a resident of Banilad Cebu City Philippines, after having been sworn to in
accordance with law do hereby depose and states:

1. That I am previously employed at MALLEUS ANIMAL CLINIC doing

business at Door 3 Arrozal Bldg., El Rio Village, Bacaca Road, Davao City
Philippines from April 21, 2014 up to September 29, 2016 working as a
veterinarian;

2. That MALLEUS ANIMAL CLINIC where I was previously working was just a
small clinic employing only two individuals including myself;

3. That by the time that I was employed in the aforesaid clinic, I did not sign any
working contract with them considering that it is not their practice to have a
written contract on their hired employees for the reason that they have only two
employees to run their clinic;

4. As such, I cannot submit a written employment contract from the aforesaid

clinic due to the fact that at the time I was working with the clinic, there was no
employment contract being signed by us;

5. That I execute this Affidavit inorder to attest the foregoing facts and for
whatever purposes that this Affidavit may serve.
