AFFIDAVIT OF EXPLANATION

I, SINSP JOY MATOCINOS CAWI, of legal age, married, a resident of Purok 4-East Gibraltar Baguio
City, a bonafide member of Bureau of Fire Protection and currently the Fire Marshal of the Municipality
of Calauan, Laguna, do hereby depose and state that:

1. That last February 15, 2024, my daughter namely, Miguel M Cawi III, was diagnosed with Bilateral
Pneumonia as shown on her x-ray results. Since then, she is on medication and is being closely
monitored at home;

2. That she was not advised by her doctor to be confined in the hospital to protect her mental health
since my daughter has a trauma in hospitals;

3. That last March 11, 2024, she had undergone another X-ray and the result showed Right Lobe
Pneumonia, showing some signs of improvement, however the doctor said that my daughter's condition
might worsen if she had another episode of difficulty of breathing with fever.

4. Thus last March 26, 2024 during nighttime, when she had a fever and cannot breathe properly, I have
to closely monitor her condition to avoid any severe complications that may worsen her health.

5. That I am personally monitoring my daughter's condition based on her doctor's instruction and
prescription till present which caused my absence during ORSITE last March 27, 2024.

6. Attached herewith are her X-Ray results and Medical Certificate dated March 28, 2024, showing
Bronchial Asthma in Acute Exacerbation.
