Head Room and Nurse discussing about discharge planning which will given to the
patient shown Ny. Adel with diarrhea and vomiting diagnosis

Nurse : Good Morning, Miss Titi. I’m Shifa as a nurse.

Previously, I would like to submit a form to the patient discharge
planning plans Ny. Adel in the 402.
From the observation, the patient's condition has improved.
Nice patient's physical condition, is no longer dhiarrhea and vomiting,
fever again and did not limp anymore. In terms of nursing care the
patient was able to go home today. I plan to give to the Ny. Adel
discharge planning. How do Miss Titi ? Did the mother approve?
Perhaps Miss Titi can see the format of discharge planning plans that I
have created.
Head Room : What is focused on discharge planning this?
Nurse : It will be given counseling about prevention of diarrhea and
vomiting, what needs to be considered when the patient comes home
and prepared leaflets which can be taken home by the patient.

Head Room : Good if so then we discuss it again with the doctor today.

The patient had undergone hospitalization for a week in the hospital and today do visit
regularly by a doctor with the head of the room and the nurse on duty in the morning
shift.

Nurse : Good morning Madam. What is the situation today madam?

Patient : Morning. Already passable Nurse. I have no fever anymore.
Nurse : How to sleep last night madam?
Patient : Sleep tightly nurse.
Nurse : It seems Madam had improved conditions, today there will be a
visit from Doctor. As with previous visits, Madam will check health
condition from doctor.
Doctor : Good morning, madam. I have heard from nurses who treat you
that you have started to improve conditions and the results of the
laboratory have also shown a good development. How diet, madam? Is
this morning the food has been eaten?
Patient : Today I consumed a serving, doc. Different from the other day,
I eat because the food did not feel uncomfortable to swallow food.
Doctor : Already good diet. Later more drinking water as well, in order
to keep the body temperature normal and to improve the immune
system as well madam. Overall from my observations, madam've been
progressing pretty good health. Just need a lot of rest first in a few
days.

situation and plan further therapy.

Head Room : Doc, the patient madam adel, what should be planned for the
home, from the results of observations conducted nurses, the patient's

improvement. Platelets and hematocrit have also been normal. What

implemented to the patient and make sure if the patient also have to
understand what to do at home
Head Room : Yes Doc, so how can the patient be discharged today?
Doctor : Yes, can. Adel patient can go home today, I will make his return
license and prescription drugs should be administered to the patient
