FACTS OF THE CASE:

Teresa R. Celestino, 50 years old, married and a resident of Maasin City,

Southern Leyte, personally appeared at this station and requested to put on record that
on or about 2:00 p.m. on December 18, 1992, while she was visiting her son, Ceres L.
Celestino, and his family in Urgello St., Cebu City, her daughter-in-law, Sunshine R.
Celestino, slapped her and scratched her face during an altercation which ensued
between her and her daughter-in-law and such incident was reported by her son to the
police.
