1 Job plan and pre-start inspection Nil.  Prior to use, conduct a visual inspection of the Supervisor
welder and associated equipment. Ensure all
leads and the hand piece are insulated and has Boilermaker
been inspected and tagged within 3 months by
an authorized person.
 Any faults identified need to be reported to the
Site Manager and the welder NOT used until all
faults have been repaired.
 Protective leather blankets and Fire
Extinguisher to control sparks & prevent
damage to adjacent areas.

2 Position welder and materials for use Manually Handling, potential sprain and strain  Park work vehicle and deliver materials as C 3 13 Supervisor
injuries close as possible to the work area to reduce the
travel distance to carry the equipment / Boilermaker

materials.
 Organise mechanical lifting aids (crane,
forklift, trolley, hand jack, etc) to be available for
handling heavy and/or awkward materials.
 Seek assistance and utilize share lifting
when handling heavy and/or awkward materials.
 Ensure the travel path and set down area is
Slip, trip and fall. clear of slip and trip hazards.
Electrical fault, potential electrocution  Ensure the welder is plugged directly into
the site electrical supply board (fitted with an
RCD). If this is not practical, ensure a portable
RCD unit is used and tested before using the
welder.
 Keep all electrical leads clear of water.
 Ensure equipment is used as per the
manufacturers recommendation, including the
use of suitable welding rods with selected
welder amperage.
 Ensure good contact of Earth Lead.
3 Welding Welding flash and burns  A Thiess Hot Work Permit must be obtained C 3 13 Supervisor
before moving the welding equipment; and no
welding is to take place without a Thiess Hot Boilermaker
Work Permit.
 Ensure all PPE is worn, including a welding
shield, leather gauntlets, steel capped boots,
long sleeved cotton shirt and long pants.
 Where required, welding screens need to be
erected to reduce the risk of welding flash to
other workers.
 Erect signs “CAUTION WELDING IN
PROGRESS” at the entrances to the welding
work area.
 Ensure there are no workers below the
welding area. If workers do enter the area
below, welding is to stop immediately.
 Care is needed when handling welded
materials to prevent the risk of burns.

controlled to ensure an acceptable level of risk is maintained?
 Are there any mechanical aids or special tools / equipment that need to
be made available?

Medical Treatment Injury / Restricted Work injury; plant damage < $100K; interruption to
2 Minor
productivity; environmental release immediately contained
Extensive injury (LTI), plant damage $100K - $500K; loss of productivity 1 to 5 days; loss of

QUALITATIVE MEASURES OF LIKELIHOOD / PROBABILITY

LEVEL MEASURE DESCRIPTION GUIDE

A Common The event is expected to occur in most circumstances Once per week
Has Will probably occur in most circumstances
B Once per month
Happened
Could
C Might occur at some time Once per year
Happen
D Not likely Could occur at some time Once per ten years
Practically May occur only in exceptional circumstances Once per life of facility
E
impossible (10 – 30 years)

H = High risk Immediate action required, activity must not start or be stopped if started. Highest level of Management needs to be involved.
M = Medium risk Monitoring controls are required. Management responsibility must be specified.
L = Low risk. Identify hazards and implement controls. Manage by routine processes. Ie inspections / observations
