IPTeC INDIANA PHARMACY TEACHING CERTIFICATE PROGRAM

Final Teaching Portfolio Checklist

All portfolios must be presented via a website (Wix, etc.) and NOT through a file sharing site (Dropbox,
Google Drive, etc.).

Teaching portfolio created and disseminated via a website (not file sharing site)
Midpoint portfolio evaluation by mentor
Completed teaching log meeting minimum hour requirements below
Updated Curriculum Vitae reflecting teaching experiences throughout program
Teaching philosophy (philosophy should be displayed in evidence of teaching below)
Evidence of teaching: including handouts and/or other materials from each teaching experience
both didactic and experiential which reflect the teaching philosophy
o Didactic (2 hours): instruction to health professionals and/or health professional students
o Experiential: APPE or IPPE rotation teaching
 Didactic (6 hours) – facilitation of or assessment of learners delivering topic
discussions, journal clubs, presentations, debates, etc. [presenting to a preceptor
alone without learners (APPE or IPPE students or other health professional
students) will NOT count]
 Precepting/co-precepting (6 hours): patient care discussions, patient care
activities, project management, etc.
o Small group teaching (3 hours): lab, recitation, and/or small-group teaching within a
classroom setting with students (not in clinical setting)
Faculty/preceptor evaluations of both didactic and experiential teaching experiences
Student evaluations of both didactic and experiential teaching experiences (If more than 2 student
evaluations for a particular activity were collected, evaluations should be compiled into a summary
document by the participant)
Narrative self-assessments/reflections for each teaching experience category (didactic, small
group, and experiential)

*All criteria must be met from portfolio submission for a participant to be awarded the teaching certificate
