Kaballero St., Pilar Village, City of Las PInas, after having been duly sworn to in
accordance with law, do hereby depose and say that:

1. I am holder of a Philippine Passport with no. EZQ1234 issued by the

Department of Foreign Affairs;
2. Sometime in June 2017, while I was at the vicinity of Robinson’s Place
Manila, through inadvertence, I lost my envelope wherein the
abovementioned passport was placed;
3. I exerted all efforts to locate the same but to no avail;
4. I am executing this affidavit to request for the issuance of a new one to
replace the lost passport and to attest to the truth of all foregoing
statements and for whatever legal purpose it may serve.

II. QUIT – CLAIM IN LABOR CASE

I, A, being the employee of ABC Corporation (hereinafter the Corporation),

hereby acknowledge the receipt of Php 350,000 from the Corporation in full
satisfaction of my claim against the Corporation for unpaid salaries and other
benefits arising out of my employment with the Corporation. I hereby renounce
any and all claims I have against the Corporation arising out of or connected with
my employment therein. This waiver and quit-claim is executed freely and
voluntary without any duress exerted upon me by ABC Corporation or any other
