TRADING PREMISES ROUTINE INSPECTION FORM

Trading Name: African Threads.

History of outbreak:

 13 workers from the same factory were affected by diarrhoea as of (17/09/19) until day of
inspection (18/09/19).
 The employees received medical treatment from Poly Clinic on 17 September 2019.
 They were all diarrhoea cases and none were recorded before.
 All patients were not sharing food or water. They don’t live close by and no interaction
amongst them.
 At their homes there are no other sick people.
 Nature of Complaints: All 13 patients have diarrhoea cases from same company.
 All were females including 9 students on attachment and 4 general workers.
 No sharing of food or water amongst themselves.
 At home there are no other sick persons recorded.

Number of Contacts:

Thirteen workers were recorded by Poly clinic from African Threads.

Thirteen employees recorded out of one hundred and fifty employees.

Signs and symptoms:

Stomarch pains.

Diarrhoea without dehydration.

Weakness

Loss of appetite

No vomiting.

Possible cause: Most of them are suspecting water which was used a day before.

There is a dry well so the company always buy water from bulk water suppliers.

There is limited water supply where some workers complained that water was only given the
whole day on the day in question.

Maybe from a contaminated food or water.

A bacterial infection.

Medical Examination Certificates:

No certificates were administered for any staff members from the kitchen.

Source of water:

Supplied by Bulk Water Suppliers Stand number 22472 Riverside, contact 0777641275 or
0733064078 were they get 600 litres every delivery.

They have a well at the premise but for the time being it is dry.

Source of food:

Tomatoes and onions supplied from the garden from Goromonzi Turn-off.

Vegetables supplied direct from the garden.

Mealie meal supplied from TM and Choppies.

Findings:

Kitchen:
Workers do not have aprons and hats when cooking.

The same clothes they wear when cleaning toilets are the same they use in the kitchen.

Canteen:

Floors need to be polished and properly cleaned.

All canteen chairs and tables should be cleaned at all times as they were very dirty.

Clean water should be provided for cleaning plates after use.

Boiling Urns should be cleaned and separated from being used for workers tea and water for
cooking.

Waste disposal:

Refuse bins:

One bin used for the whole premise for refuse.

More refuse bins should be provided.

Hygiene facilities:

Male toilets: Two toilets are being used for the whole company.

Other toilets are closed due to water shortage.

Are not properly being washed due to water and suitable detergents.

Negligence of toilets.

Female toilets:

No sanitary bins in all female toilets.

Two toilets are being used by 120 females. Other two are also closed.

Pieces of cloth are being used as sanitary for the females.

Recommendations:

They should increase the amount of clean tested water supply required for the workers.

All toilets must be functional for both males and females.

Proper attire for the kitchen workers should be provided that includes aprons and caps or hats.

Proper disposal of kitchen effluent and waste.


Sanitary bins and mega bins should be provided in toilets and around the premises.

Toilets disinfectants should be provided.

All workers should exercise hygienic practice like hand washing and maintaining their toilets
and surroundings.
