Agustin to pick up my Official Transcript of Records in your school. I cannot claim my Transcript of
Records because I’m currently working as a private school teacher at Risen Christ Catholic School
located at Phase II Brgy. Anahaw, Silang, Cavite.

I’m authorizing also Mr. Agustin to sign any documents or school clearance that would help to
release my Transcript of Records immediately.
