1 PURPOSE
This procedure is to establish and maintain plans and procedures to identify the potential
for and responses to, incidents and emergency situations. It also aims for preventing and
mitigating the likely illness, injury and adverse environmental impacts that may be
associated with them.
To be used by all members of the Contractor’s Project Management Team, Supervisors
and Subcontractors personnel who have a duty of care to protect and assist all members
of the workforce in the event of any emergency situation during the construction period up
to its completion. This emergency response plan is intended for the construction period
only. In line with Company’s objective to issue a new Crisis Management System for
commissioning and operations, this plan will also be updated before commissioning starts
to cover the additional risks associated with the introduction of hydrocarbons to the plant.

2 SCOPE
This procedure applies to identify potential emergency situations associated with the all
HSE activities, products and services and respond to the situation in order to minimize the
expansion of incidents and environmental pollution in the Project Site.

3 OBJECTIVE
The object of this Emergency Response Plan is to safely evacuate all personnel from the
dangerous areas and reduce the severity of loss. And also any potential impact on the
environment. This document also addresses the recovery stage to enable recovery from
an emergency incident
In detail, the objectives this Emergency Response Plan are:

3.1 To identify the types of emergencies for the proposed process HEC intends to implement
to manage these potential emergencies including the equipment, personnel and training at
each site and the type and severity of the incident; and manage each emergency in
accordance with a detailed set of procedures that will be followed in the event of an
emergencies during the construction stage, up to mechanical completion, of the project.

3.2 Establish the roles and responsibilities of Contractor and Subcontractor staff on site during
an incident or emergency.

3.3 Ensure that all emergency response activities are undertaken in accordance with
Company Crisis Management System for Construction and including the immediate
notification of events and, where necessary supporting Company’s Emergency Response
Team as required.

4 INTEGRATION WITH COMPANY CRISIS MANAGEMENT SYSTEM


All emergency incidents will be immediately notified to the Company’s construction
Manager or Company’s on site representative immediately. Depending on the severity of
each incident Company’s Construction manager will decide whether or not to mobilize

Company’s local incident management team to provide additional support to Contractor’s


emergency management efforts.

5 DEFINITIONS

5.1 Incident
Any event that causes or has the potential to cause (Near Miss) personal injury,
damage/loss of assets, environmental damage or harm to the health and/or safety of any
individual.
Incidents do not include operations, maintenance, quality or reliability issues which had no
Health Safety Security and Environment consequence or potential. Incidents do not
include degradation or failure of plant or equipment resulting solely from normal wear and
tear unless this results in injury to persons, damage to the environment, asset loss or
reputation damage.

5.2 Crisis
A crisis is an incident (either a situation or an event) that has the potential to impact the
wider organization in terms of safety of life, continuity of business or damage to reputation;
where there is a degree of complexity so that it is unclear what action should be taken; and
where extraordinary resources are required to bring the incident under control and
manage it to a satisfactory outcome.

5.3 First Aid


Any one-time treatment and subsequent observation of minor scratches, cuts and e.g.
such treatment and observation are considered First Aid, even though provided by
qualified medical personnel.
The fallow up check should be done daily basis for the treatment.

5.4 Emergency
Defined as any unplanned occurrence resulting in, or having the potential to result in death,
serious injury, and threat to the safety of the public and/or company/contractor personnel,
major or significant damage to company property and operations, or pose a potential
impact to the environment

6 RESPONSIBILITY

6.1 Project Manager


Project Manager is responsible for the overall preparedness of Contractor to respond to all
emergency. The Project Manager shall support this Emergency Response Plan with
regards to the following:

• Ensuring adequate resources and trained personnel, medical and other emergency
facilities needed to implement and execute the Emergency Response Plan are
available during the construction period.
• Provision of adequate resources in terms of personnel, time and finance for
implementation of the requirements for providing an emergency response service on
site.
• Active participation in the scheduling and enactment of emergency response drills,
exercises and training.
• Shall monitor the arrangements through the HSE Manager to ensure that the
Emergency Response Procedures and their implementation are appropriate for the
needs of the project.
• Ensuring that after an emergency and before routine activities continue, all managers
and line supervisors shall assess facilities for damage and loss and to take necessary
measures to rectify any unsafe conditions.
• Shall liaise with Company during actual incidents and emergencies and appoint
appropriate/trained logistics, admin, HSE and technical staff are to join Company’s
local Incident Management Team in the event it is mobilized
• Shall take control of the emergency situation as an Emergency Response Chief
during an emergency.
• Ensure that each emergency incident is documented, investigated, reported and that
all necessary actions are identified and closed out.

6.2 HSE Manager


HSE Manager has the overall responsibility for all HSE issues on site, is also responsible
for the development and implementation of HSE plans and procedures.
The HSE Manager shall have the following emergency response responsibilities.
• Identify plausible emergency response scenarios using the project nominated risk
assessment framework. All emergency risks to be captured and tracked in a risk
register.
• Develop, implement and maintain management plans and procedures to manage
each risk scenario identified. These plans will include procedures to guide team
members from initiation/mobilization through to response and recovery.
• Ensure a documented process is in place and operational for accounting/tracking all
personnel during and after emergency scenarios identified.
• Ensure that trained personnel, equipment and facilities needed to respond to
emergency situations are defined, identified, available and periodically tested.
• Ensure that regular exercises are undertaken to practice and test the robustness of
emergency response plans (including consideration of external communications and
involvement).
• The designation and maintenance of all Muster points throughout site
• Ensuring all emergency response equipment is well maintained, serviced and fit for
purpose.

• Ensuring the establishment and maintenance of contact details of call out personnel
and emergency services and agencies.
• Arrangement, through site management for the resources for the resources data of
personnel, equipments, materials, etc. Initiate and regularly review/update the
Emergency Response Plan
• The establishment and training of the emergency response teams.
• Setting up drills and exercises in coordination with Company’s HSE Department.
• Undertaking lessons learnt and debriefing exercises after all exercises and
emergency events to ensure all lessons are indentified and captured.
• Ensuring information and communication to staff through distribution of the
Emergency Response Plan and any documents or updates following any emergency
response exercise or after an actual incident by incorporating findings.
• Shall act as coordinate of the Emergency Response Teams in emergency situation
and exercise or drill.
In the event of any emergency the HSE Manager or a member of the HSE department
shall take the following actions with full assistance from other members of the Project
Management Team.
• Evaluate the scale of the incident and decide whether additional resources are
required to adequately deal with it.
• Immediately notify Company for the scale and nature of the event.
• Liaison with site supervision for withdrawing any permits.
• Give direction to the site emergency response teams.
• Liaise with site supervision for the mobilization of any plant and equipment necessary
for dealing with the emergency.
• Secure the area by setting up barriers or other means of preventing unauthorized
access.
• Shall lead or advise on technical aspect of emergency response requirements.
• When appropriate to do so, secure the site and commence the collection of evidence
for subsequent investigation
• Prepare a full report.

6.3 Emergency Response Team


Throughout the project execution on site, the HSE Manager shall develop the Emergency
Response Team (consisted of long-term experienced and trained personnel, including fully
trained on scene commanders) to be able to deal with emergencies listed above that arise
in the workplaces. The Emergency Response Team will also cover all locations including
the project site, the temporary accommodation camps, the construction site and all other
project locations.
Subcontractors shall provide qualified personnel and emergency equipment.
The maintenance of firefighting equipment shall be the responsibility of the designated
safety officer(s) of the area. If fire equipment is used, the HSE Department shall be

responsible for recharging or re- equipping


• Emergency Response Team is responsible for conducting rescue operation in
accordance with established procedures.
• Emergency Response Team shall be fully trained in different rescue techniques and
procedures, e.g. removing, securing a victim from the scene of incident, first aid and
etc.
• The Emergency Response Team shall be involved in training programs and
drills(established by the HSE Manager) which shall include , but not be limited to the
following:
- Fire fighting training;
- Evacuation procedures;
- Fire wardens/ Marshall;
- Fire prevention training;
- Collapse of excavation;
- Vehicle incident;
- Gas release;
- First – Aid and Rescue;
- Crane tipping over;
- Accidental or improper explosive detonation;
• The team shall be responsible for recovering any casualties and properly transporting
them to the Medical Center.
• The team shall also assist the doctor and nurse on duty in rendering medical aid to
injured parties. If nursing staff is not available the leader of the team shall take charge
by administering first aid and sending injured or sick person(s) for further treatment to
the medical center or local accident and emergency hospital.
• The Medical Center shall be managed on emergency situations, and the actions
listed below should be followed.
- Take care not to become a casualty themselves.
- At least a doctor and nurse to be placed as follow-up against any incident it
might happen during the execution work.
- Move the casualty only to prevent further injury.
- Check the heart and breathing; giving emergency CPR as necessary.
- Stop any bleeding; raise the injured part and apply pressure.
- Keep the victim warm and reassured.
- Rescuers/ First aiders etc. should not put themselves in danger. Another
casualty often reduces the chance of providing rapid assistance to the first
injured person.

6.4 Camp Chief (Security Manager)

• In emergency situation in Camp area, Security Manager/ Camp Chief are responsible
for ensuring the Medical care of the Camp and people shall be gathered to the Muster
Point.

6.5 All Personnel


• All personnel should familiarize themselves with the locations of all emergency exits
from their work area and the location of their nearest Muster Point.

7 GENERAL
Any emergency occurring on construction sites has serious consequences. People may
be injured or fatalities may occur, buildings/structures and equipment can be damaged or
destroyed. Such situations demand adequate response, with rescue and relief measures
to handle such events safely, quickly and efficiently. It is therefore necessary to ensure that
all persons on site are aware of their responsibilities during an emergency response and
duties of key personnel must be established.
Analysis of the effectiveness of the emergency response procedure will be carried out by
practical training complete with a program of regular drills and exercised at least every Six
(6) months.
The procedures, in addition to setting out the duties and responsibilities of key personnel
and individuals, shall also describe the system of evacuation which shall include
accounting for all site personnel, fire fighting, rescuing and handling other types of
emergencies. It shall also indicate the availability of facilities such as location of firefighting
equipment, muster points , exits and access routes and the procedure to arrange
assistance if necessary from any outside agency.

8 PROCEDURES
HSE Manager shall identify the potential emergency situations through;
• Risk assessments
• Environmental aspects identification,
• The experience of previous incidents and emergency situations, and
• Legal or other requirements.

8.1 Emergency Response Plans


• HSE Manager shall prepare the Emergency Response Plans for the identified
emergency situations.
• Project Manager shall approve the Emergency Response Plans.
• The Emergency Response Plans shall give an outline of the actions to be taken when
specified emergency situations arise, and shall include;
- Identification of potential Incidents and emergencies,
- The most likely type and scale of an emergency situation or incident that might
occur.

- Identification of person who will take charge during the emergency,


- General duties of all personnel during the emergency,
- Responsibilities, authority and duties of personnel with specific roles during the
emergency,
- Interfaces with external emergency response services.
• The Emergency Response Plans shall address the following scenarios as a
minimum;
- Site injury,
- Motor vehicle accidents,
- Fire and explosion,
- Loss of power or communications,
- Loss of containment,
- Damage to oil and gas treatment facilities or pipelines if relevant,
- Environmental emergencies, spills and unplanned releases of Hazardous
waste,
- Loss of radioactive material or explosive material,
- Missing person, or man overboard, as relevant,
- Security breaches and sabotage, civil disorder, war,
- Earthquakes, hurricanes and other natural events, and
- Hazards associated with simultaneous operations.
• If possible, The Emergency Response Plans shall include the information of the
external emergency services or a client. That will provide the possible scope of their
involvement and the information as they may require to facilitate their involvement in
the following emergency response activities;
- Communication with statutory bodies,
- Communication with neighbors and the public, and
- Protection of vital records and equipment.
• HSE Manager shall review the Emergency Response Plans annually. If required,
Emergency Response Plans shall be revised and inform affected personnel and an
organization.
• HSE Manager shall ensure appropriate communications take place for each
employee covered by the Emergency Response Plans at the following times;
- During HSE Orientation Course,
- Upon initial development of the plan,
- When an employee’s responsibilities or designated actions under the plan
change,
- When the Emergency Response Plan is revised, and
- During table-top exercises and simulated drills.

8.2 Emergency Equipment


• The Emergency Response Plans shall also identify the emergency equipment
needed. The emergency equipment shall be provided in adequate quantity and
tested at specified intervals for continuing operability.
• Emergency equipment includes, but not limited to, the following;
- Alarm system,
- Emergency lighting and power,
- Evacuation route,
- Fire-fighting equipment,
- First aid equipment, and
- Communication facilities.

8.3 Emergency Response Drills


• HSE Manager shall ensure that the emergency response plans, for various scenarios,
are tested at least once every three (3) months or sooner depending on changes in
identified hazards as construction site develops and progress. If required, Client or 0
external emergency services may be invited to participate in emergency response
drills.
• HSE Manager shall review the results of the Emergency Response Drills about the
suitability of followings;
- Scenarios of the Emergency Response Plan
- The Emergency Equipment
- Implementation of employee’s responsibilities or designated actions
• HSE Manager shall report the results of the emergency response drills to Project
Manager
• Any actions that are identified as being necessary after the review shall be
implemented accordingly by HSE Manager, such as followings;
- Change of the Emergency Response Plan
- Education or training for employee
- Change or supply of the Emergency Equipment, etc.
• HSE Manager shall maintain the results of the emergency response drills.

8.4 Evacuation Procedures


Anyone working in Operational Areas shall follow Company’s Evacuation Procedure
In the event of a fire, major plant failure, explosion, bomb threat or the need to evacuate
the plant, the actions listed below should be followed.
• On the continuous sound of the alarm siren (bells), STOP all activities and vacate the
building or area without delay, by the nearest exit
• Move quickly, but do not run.

• Do not return to a work area to collect belongings.


• Keep left in corridors and stairs.
• Do not overtake others along the route.
• Assemble in the designated Muster Point.
• At Muster Point warden reports to HSE Manager.
• Do not enter the building or work area under any circumstances until the all clear
signal is given.
All new staff, as part of their induction, shall be given instructions on the evacuation
procedures for the site.
All visitors and Subcontractors are to be advised of the Site's Evacuation Procedure and
the location of Muster Point when they are admitted to the site.

8.5 Raising the alarm and communication:


Communication is a critical factor in handling an emergency. To control the situation by the
earliest possible action, any employee must be able to act and raise an emergency alarm.
• Raising Alarm shall be reported to HSE Manager promptly by the equipment at site
such as a phone or radio.
• For fire control at the work place, areas will to be sub-divided into different zones with
each zone having a color code. In this situation there may be more than one Muster
Point.
• The muster point shall also be clearly indicated in the accommodation areas with fire
instructions.
• Procedure on emergency
- All personnel shall vacate the room or work place. If it is safe and prudent to do
so, before leaving the working place the individuals concerned shall leave the
work place as secure as possible to avoid further damage or injury. e.g.
switching off of welding machines, closing gas cylinders, turning off running
machines, gas burners, any electrical light or appliances in their rooms.
- All personnel shall assemble at the muster point according to the color code of
their work place.
- Fire wardens shall provide guidance and assistance for mustering at the
correct muster point.
- While responding to a fire alarm, kitchen staff shall ensure that all burners and
other fire hazards have been turned off. Similarly other personnel in residence
shall ensure that they leave everything in their rooms in safe condition.
- All the members of respective teams shall report to their leaders. The leaders
shall take direction from the HSE Manager.

8.6 MUSTER Points


Designated Muster Points shall be marked on the site layout maps. These map and
Evacuation Procedure shall be displayed on Notice boards throughout the site.

Muster point wardens should be appointed to manage and be responsible for operations
at each muster point.

8.7 Incident Completion


All clear signals shall be sounded by the Contractor's HSE Manager following a thorough
examination of the area, initiated by the Contractor's HSE manager or an appointed team
leader.

8.8 Specific Emergency Response Procedures

8.8.1 Explosions
In the event of an explosion at the site:
• Initiate evacuation procedure.
• All staff to assemble in Muster Point Area.
• Account for ail personnel, visitors, and contractors.
• If fire, initiate fire procedure.
• Control officer to initiate Emergency Operation Plan to minimize plant damage, if able
to do so safely.
• Await further instruction from Site HSSE Manager.

8.8.2 Chemical / Fuel Spillage


In the event of a Chemical /Fuel Spillage or leak, the priority actions listed below should be
taken:
• Ensure personnel are safe and clear the area.
- Be implemented first, to check MSDS, Material Safety Data Sheet as to hazard
and dangerous materials.
- If personnel are injured and can be removed from the area safely, do so.
• If it is safe to do so, stop the flow of leaking material.
- Reposition the drum to stop the flow.
- Shut off the source of the leak.
- Equipment filled with the material that is found leaking should be removed from
service as soon as possible.
• Contain the spill.
- Bund and protect all open grates, sumps, manholes that discharge to into the
storm water system.
- If the chemical has entered the drains, advise supervisor and environmental-
Supervisor immediately.
- Dyke major spills with soil or other material.
- Prevent spilled material entering drains, waterways or on to the ground.
- Where it is not possible to remove equipment from service immediately, some

means of collecting the leaking material should be used, such as metal trays,
buckets, polyethylene sheeting.
• Protection of Personnel
- Personnel entering the leak or spill area shall be provided with and use
appropriate protective equipment, as set out in the Material Safety Data Sheet
(MSDS) for that particular chemical.
• All non-essential personnel shall be kept out of the immediate leak or spill area.
Clean up
- Minor spills or leaks can be cleaned up using absorbent material, sand,
sawdust, or kitty litter. Check MSDS to see what the correct material is.
- All liquid and contaminated material must be collected for disposal.
- Transfer contents of leaking drum to new clean drum.
- Ensure collected material is placed in a container that will not leak during
transportation off site for disposal.
- Check MSDS that disposal route is appropriate, e.g. to landfill.

8.8.3 Vehicle Accident.


In the event of a vehicle accident, the actions listed below should be followed
• Send for help if anyone is injured.
• Try to prevent other accidents from happening by warning other vehicles of the
vehicle accident by placing accident hazard warnings in the area. Stand clear of
vehicle or roadway if there is other traffic.
• Administer first aid to injured person.
• As soon as possible, record details of the accident.
• Report accident to Site Manager as soon as possible.

8.8.4 Fire on the Site


Safety Equipment will be maintained in good working condition by systematic inspection
and test as required by Company and specific procedures.
The permanent fire-fighting system shall be fully operative prior to beginning the Start-up
activities.
Furthermore each component as well as portable fire extinguishers, and other safety
systems shall be inspected and tested regularly to ensure availability at all time. Project
personnel will use their individual judgment as to whether it is safe to attempt to fight a fire
with available fire extinguishers and they should not hesitate to leave the fire scene if they
feel they are in imminent danger.
Any employee detecting flammable gas or noticing a fire should:
• Immediately operate the nearest fire alarm if possible and report the incident by one
of the available means of communication. Speak clearly and relay the following
information:

- Name and belonging organization
- Time of Incident
• Notification will be activated by one of the available means of communication.
- Manual Fire Alarm Point
- Mobile/ radio
• Verbal announcement
- The person detecting the fire must than alert personnel in the immediate area
to the fire hazard.
• Once the fire has been extinguished HSE Department will investigate the incident
and issue a detailed written report on the incident.
In the event of a fire emergency the Site HSE and Security Manager (or his Deputy) will
proceed to the scene of incident. Thus he will take the following actions:
• Coordinating activities of the Contractor HSE personnel
• Liaise with subcontractor on the mobilization of any equipment that might be needed
during the fire emergency
• Verifying the entire workforce has vacated the area and is at the muster points
• Request additional resources
• After the incident, make a full report.

water drains.
Bund all storm water opening in area.
Collect chemical/Oil from equipment

IF unable yourself, get a fellow worker to


report the incident

Report the Incident Foreman and Supervisor


Quickly. If at a time no foreperson or supervisor
present, go to emergency contact list.

Secure the Area. Keep non-essential persons away.

Clean up the Spill Check MSDS as to clean up


procedure. Collect spilled material
with non-combustible absorbent
material. Place contaminated
material in drums for safe disposal.
