Thank you very much for accepting and partaking in this study, “Holistic Nursing Care
Practices Among the Medical Ward Nurses: Basis for the Development of Holistic Nursing
Care Approach Scale”. The study is a two-part series of questions that will collect
information regarding the topic of this study. Part I focuses on creating the participant profile,
Part II discusses about various holistic nursing practices, all questions will be answered
through a Likert Scale (or rating from 1-4).
Rest assured that all information collected, from all individual identities and responses,
in this research process are kept confidential, only the consolidated final results will be

PART II: HOLISTIC NURSING PRACTICE


INSTRUCTIONS: PART 2 is composed of 30-questions. Focused on the various
holistic nursing care provided to the clients. There is no right or wrong on the responses in
this survey. Please rate the following statements using the following scale: 1- Never, 2-
