 CRITICAL INFORMATION REQUIREMENTS (ICS FORM 202b)

Purpose: The Critical Information Requirements form supplements the ICS 202 form by documenting the
SMEEC/IC strategic direction and guidance through Critical Information Requirements for use
during the next operational period.

Preparation: The Critical Information Requirements form is completed and/or updated by the Planning
Section and approved by the SMEEC/ IC at the SMEEC/IC Objectives Meeting. The form is
used in preparing the Incident Action Plan.

Distribution: The Critical Information Requirements form may be reproduced in the IAP and should be given
to all supervisory personnel at the Section, Branch, Division/Group, and Unit levels. The form is
saved to NEMO.

Item Title Instructions

Version # Enter the version number for the form.
Incident Name Enter the name assigned to the incident.
Operational Period Enter the Operational Period for which the form applies, comprising time (24 hr)
and date (DD MMM YY). For example, 0800 15 Oct 19 to 0800 16 Oct 19.
Critical Information Enter clear, concise statements of critical information requirements for the Requirements
Requirements response. These requirements are for the incident response for this operational period
and for the duration of the incident. Listed in order of importance.
Approved By Enter the signature and date of approving parties.
Prepared by Enter name of the person preparing the form. Enter time (24 hr) and date (DD MMM YY).
