INSTRUCTIONS TO CANDIDATES

1. ENTRY FORMS
Please read these instructions carefully before completing Entry Forms. The closing date for submitting entries is February
29, 2012.

2. FEES
The fees for the examinations are as follows:
Each Ordinary Level Subject Fee - $404.00
Each Advanced Subsidiary and Advanced Level Subject Fee - $713.00
General Studies Subject Fee - $676.00
Local Fee - $ 8.00

Fees may be deposited at the Revenue Collection Unit Ministry of Education or at any District Revenue Office using the
Special Deposit Voucher attached.

FORM A : Head 111/73 – for Subject Fees

FORM B : Head 07/01/ED1/004 – for Local Fees

4. SUBMISSION OF APPLICATION

Entry Forms may be submitted at the following venues:

I. Examinations Section, Ministry of Education, Alexandra Street, St. Clair.
II. Education Office, 16 – 22 Sutton Street, San Fernando.
III. Education Office, Dutch Forte Plaza, Dutch Forte,Scarborough, Tobago.
IV. Education Office, Savi Street, Boodooville, Sangre Grande
V. Education Office, Corner Green Street and El Dorado Road, Tunapuna.
VI. Education Office, 14 Camden Street, Balmain, Couva.

The completed Entry Form, the official receipt for fees paid, and two (2) self - addressed envelopes 23cm x 10cm must be
submitted to any of the above mentioned offices. Stamps must be affixed to each envelope as follows:

23cm x 10cm - $1.00

The closing date for submitting entries is February 29th 2012.

An official acknowledgement of receipt of an entry submitted in person will be issued to each applicant after the entry has
been thoroughly checked and found to be in order. You are advised against posting your entry as it may arrive after the
closing date and therefore will not be accepted.

5. WITHDRAWALS

Application for complete withdrawals, subject withdrawals and subject changes will be considered up to March 16th 2012.

Please note that the Local Fee of $8.00 is non-refundable.

Applications for withdrawals received after this date WILL NOT BE considered except on the following grounds:

I. Illness
II. Death of Parent
III. Exigency of service to the Government

6. ISSUE OF TIMETABLES

Examinations are set to begin by May 10th 2012. If you have not received your timetable by April 30th 2012, please contact
the Examinations Section, Ministry of Education, Alexandra Street, St. Clair.

Candidates should immediately notify the Examinations Section of any change of address.
