AFFIDAVIT OF NO COMPLAINT

I, Ruperto C. Fajardo, of legal age, Filipino, married and with residence and postal address at
Pagalanggang, Dinalupihan, Bataan, after having been duly sworn to in accordance with law, hereby
depose and state that:

1. I am the elected Captain of Barangay Pagalanggang, Dinalupihan, Bataan;

2. Dinalupihan Water District is to develop a new water supply system at barangay

Pagalanggang which covers civil works at Tutulari Falls that includes construction of
reservoir, laying of pipelines and related interconnection works;

3. The construction of said water system is urgently needed to provide water supply to
Pagalanggang and adjacent barangays which currently suffer from very low to no-supply of
drinking water from DWD;

4. There are no concerned parties complaining on the development of the said water system;

5. I am executing this affidavit to attest to the truth of the foregoing and for whatever legal
purpose this may serve best;
