Training Registration Requirements 2018

1. Photocopy of Document of Highest Educational Attainment

The trainee applicant must have at least completed 3 year in any engineering course, completed at least
3-year technology course or graduated from any 4-year bachelor degree course. Just in case the minimum
education is not met, the trainee applicant is required to take the qualifying examination at a scheduled
date at Apex Engineering Services training center in Las Piñas City.

2. Valid Identification Card with Photo

Current company identification card or government issued identification card or passport

3. Eye Vision Examination Verification

Trainee must pass the eye vision examination verification.

4. Training Registration Form

Fill-up training registration form, affix signature and indicate the date of signing.7

5. Reservation Fee
The reservation fee shall be paid before the training proposal validity date to confirm a training seat.

6. Training Fee
The training fee shall be paid in full on or before first day of training.
