EXPLANATORY NOTE: In response to Office Memorandum No. 06 Series of 2020 dated March 16, 2020 by the Office of
the Vice President for Academic Affairs of the University, this alternative mode of teaching an assessment using online
platforms was formulated.

The undersigned patterned this outline from Associate Professor Mary Antoinette Fajardo’s work.

It is the intention of the undersigned to have this alternative mode of teaching and assessment be part of the activities
and outputs, in order for the students to accomplish the academic requirements in the subject, in strict compliance
with the guidelines set by the National Government and the various Local Government Units against the spread of
COVID-19.

It is not the intention of the undersigned that students will have to be exposed to risks in the course of complying with
this outline. Thus, students are only ENCOURAGED to comply with the online requirements via email by submitting
their outputs to the personal email of the undersigned at, albaiisalvador@gmail.com or via facebook messenger thru FB
Account, Salvador D. Alba II.

Lastly, students who are prevented to comply with the online requirements may be given other alternative activities
when classes resume after this National Health Emergency caused by COVID-19.
