This amount shall be used for the purchase of office supplies and consumables
for the printing of modules in preparation for the opening of classes in October 05,
2020, the said amount shall likewise be disbursed within the month of October 2020.

Greetings to you!
Due to a health reason, Mrs. Nenita D. Castel Accountant I of Himamaylan National

High School is tendering her application for forced leave on August 13-14, 2020 and
sick leave effective August 17 to February 28, 2021.
Mrs. Castel plays a very important role in the payroll processing and checking of School
MOOE disbursement of both Junior and Senior High School. Furthermore, she is the
signatory in all PS and MOOE banking transactions and all financial reports required by
DepEd, COA and DBM. Her absence will greatly affect the school function and
operation.
In this regard, we are asking for your assistance to address our concern in the
temporary vacant position of Mrs. Castel as Accountant I. We know that the position for
Accountant I requires one to be a Certified Public Accountant (CPA), however there is
no qualified personnel in the school that can replace her position and so we would like
to ask for the extension of services of MR. ERIC JOHN T. COÑATE, Accountant III of
the Division of Himamaylan City to help us with our operation in the absence of Mrs.
Castel to enable us to continue with the school function without any untoward delay
that will affect the school function.
