JOINT AFFIDAVIT OF TWO DISINTERESTED PERSONS

We, FLORDELIZA A. AMOR, of legal age, widower, and with residence and postal address at Purok 5 Tubod, Cangmating,
Sibulan, Negros Oriental and BELEN T. ESPERAGO of legal age, married, and with residence and postal address at Cangmating,
Sibulan, Negros Oriental, after having been duly sworn to in accordance with law, do hereby depose and say:

1. That we personally known "YOLANDA PAET TALIPAN" , legal age, married, and a resident of Purok 5 Tubod,
Cangmating, Sibulan, Negros Oriental, being her neighbors and friends for a long period of time;

2. That as neighbors and friends, we often participated in family discussions, together with the other members of her
household;

3. That we came to know for a fact that in her Marriage Contract, her middle name was erroneously entered as "V. or
VICENTE", instead of "P. or PAET" which is her correct middle name;

4. That unfortunately no one took notice of the discrepancy in her middle name and the error was never corrected;

5. That the name "YOLANDA PAET TALIPAN", as appearing in her Certificate of Live Birth issued by Philippine Statistic
Authority (PSA), and the name "YOLANDA VICENTE TALIPAN" as appearing in her Marriage Contract, refer to one and
the same person;

6. That we are executing this Affidavit to attest to the truth of the foregoing facts and for whatever legal purpose it may
serve.
