Dear Hon. Pelopero,

Greetings!

I would like to inform you that MR. IRENEO LUMAYNO former Sangguniang Bayan
Member of your Municipality has an outstanding balance with Pototan Farmers Multi-
Purpose Cooperative (PFMPC). Despite numerous reminders, telephone calls requesting
payment of his account, the debt remains owing. I am writing requesting your good
office to deduct his outstanding balance with Pototan Farmers Multi-Purpose
Cooperative (PFMPC) from his terminal leave benefits, as based on signed
Memorandum of Agreement (MOA).

I enclose a copy of signed MOA, summation of his account including the copy of ledger
of his last payment dated May 2, 2019 for the total outstanding balance of
₱225,454.00.

If this matter is not resolved, I reserve the right to commence legal proceedings to
recover the debt without further notice and this letter may be tendered in court as
evidence of failure to pay. Legal action may result in you having to pay legal costs,
interest and could impact his credit history.

In this regard I am appealing your good office to act on this matter. Thank you and
