TRAINING CONTRACT

This Training Contract made and executed this ___ day of _________ _.
Zamboanga City, Philippines by and between:

___________________, a government-owned or duly recognized company/ corporation

created and existing under the laws of the Republic of the Philippines with office address at
___________________________, referred to as “Training Institution”.

and

_____________________, of legal age, Filipino, resident of ______________________,

referred to as “student-trainee”.

Training Contract Obligations

A. Training Institution shall agree to be bound by the terms and conditions stipulated
hereunder:

1. Ensure that the student-trainee is given activities and tasks that will develop his/her
skills, work attitudes and creative abilities while undergoing On-the-Job Training.
2. Designate a Training Coordinator or Immediate Supervisor to orient student-trainee on
the rules and regulations especially on safety and security precautions and to oversee
the work of the student-trainee.
3. Communicate with the Director, On-the-Job Training and Apprenticeship Placement or
the OJT Coordinator any feedback, problems or incident regarding the student-trainee.
4. Assess the performance of the student-trainee by filling-up the Evaluation Sheets and
On-the Job Training Accomplishment Report at the end of the OJT.
5. Student-trainee shall not be allowed to render more than eight (8) hours a day without
his/her consent and his/her parent/s/ guardian/spouse consent.
6. Student-trainee shall not be allowed to perform tasks which will involve high physical
risks.
7. Reserves the right to terminate the training of the student-trainee who consistently
violate the rules and regulations of the training institution and such other just and other
valid causes provided for by law.
8. Free from all or any liability, should the student-trainee suffer illness or sustain injury
while inside the company premises if such injury was sustained due to the failure of
latter to observe and follow policies, rules, regulations, safety procedures and the likes.
9. Issue a Certificate of Completion to the student-trainee upon successful completion of
the training.

B. Student-trainee shall undertake the following responsibilities:

1. Perform carefully the activities/ tasks entrusted to him/her as part of the OJT by the
Designated Training Coordinator/ Immediate Supervisor or any other person
authorized to give them instructions.
2. Observe and follow all the Training Institution’s rules, policies, regulations, safety
procedures and the likes for the effective, safe and successful completion of the OJT.
3. Regularly report to the Training Institution and notify the Training Coordinator/
Immediate Supervisor or any key personnel in case of inability to do so for any reason
whatsoever.
4. Student-trainee is not entitled to any compensation or honorarium from the training
Institution since the OJT is solely for the purpose of undergoing a training as part of
the school curriculum and/or as requisite for graduation.

5. Not reveal any business nor trade secrets that have come to their knowledge in the
course of the training.
6. Student-trainee will only be allowed to leave the training premises during the training
period for reasons of health and other duly verified and valid concerns from
parent/legal guardian/spouse and upon notice to the “Training Institution”.
7. Student-trainee will be issued a Certificate of Completion ONLY upon completion of
the required number of hours as stipulated.

IN WITNESS WHEREOF, the parties have signed this Training Contract on the date first
hereinabove written in Zamboanga City, Philippines.
