That I, SEGUNDO D. HIDLAO, of legal age, married, Filipino and a resident of Brgy.
16, Washington Drive, Legazpi City, do hereby by these presents, NAME,
CONSTITUTE and APPOINT my DAUGHTER, HIYAS HASMIN HIDLAO-CARO, of
legal age, married, Filipino and a resident of Brgy. 16, Washington Drive, Legazpi City,
to be my true and lawful attorney-in-fact, for me and in my name, place and stead, and
for my own use and benefit, to do and perform any and all of the following acts and
things:

1. That I am retiree with an approved retirement claim under the

Government Service Insurance System to with:

REF CLAIM NO.: RA 8291S04700092682200200227
POLICY NO.: CM00005101765
ID NO.: 56081602447
2. That my retirement claim in the total amount of ONE HUNDRED
SEVENTY THREE THOUSAND EIGHTEEN PESOS AND FORTY
TWO CENTS (Php 173,018.42) has been approved by GSIS, a copy
of the said approval is attached as ANNEX “A” ;
3. That I NAME, CONSTITUTE and APPOINT my DAUGHTER, HIYAS
HASMIN HIDLAO-CARO to process my RETIREMENT CLAIM with
the GOVERNMENT SERVICE INSURANCE SYSTEM OFFICE and to
the UNION BANK OF THE PHILIPPINES, or LAND BANK OF THE
PHILIPPINES;
4. To sign and receive any papers from the GOVERNMENT SERVICE
INSURANCE SYSTEM OFFICE and to the UNION BANK OF THE
PHILIPPINES, or LAND BANK OF THE PHILIPPINES; in claiming my
retirement benefits;
5. To make, sign, execute and deliver, for and in my behalf, any
documents which may be necessary for the approval of my retirement
benefits in the GOVERNMENT SERVICE INSURANCE SYSTEM
OFFICE and to the UNION BANK OF THE PHILIPPINES, or LAND
BANK OF THE PHILIPPINES ;
6. To receive and encash the proceeds of my retirement claim and to
sign/endorse the same;
7. To deliver to me the proceeds thereof;

HEREBY GIVING AND GRANTING unto my Attorney-in-Fact full power and

authority to do and perform every act and thing whatsoever requisite and necessary to
be done in and about the premises, as fully for all intents and purposes as I might or
could do if personally present, and HEREBY RATIFYING AND CONFIRMING all that
my said attorney-in-fact shall lawfully do or cause to be done by virtue of these
presents.
