MEMORANDUM OF AGREEMENT

This agreement is executed and entered into by and between:

of the Republic of the Philippines, with postal address at 73 Corrales Ave, Cagayan de Oro, 9000

non-stock organization dedicated to the promotion of mental health and prevention of mental disorders
and a member of the World Federation for Mental Health, with principal office at Dolores 9000 Cagayan
de Oro, Philippines, and represented in this instance by MS. FRETZIE ROSE DANTE, the Chapter
Executive Manager, hereinafter referred to as “PMHA CDO, MIS.OR”

WITNESSETH:

Whereas, the SESUX operates as a non-government organization comprised by

mental health issues from Cagayan de Oro to the partner institutions for mental health

“telekonsulta” as proponents of Project Sumpay. A project that aims to link people that needs
mental health support to appropriate institutions and provide mental health awareness amongst
people in the community.

Whereas, PMHA CDO, MIS.OR will provide the “telekonsulta” and other needed
services to the people of Cagayan de Oro as a requirement and duty as partner of Project Sumpay.
A project that aims to link people that needs mental health support to appropriate institutions and
provide mental health awareness amongst people in the community.
 NOW, THEREFORE, for and in consideration of the foregoing, the parties hereto agree, as
follows:

1. The SESUX and PMHA CDO, MIS. OR shall have a mutual coordination for the conduct of
Project Sumpay for 6 months starting on the first week of November 2020.

2. The SESUX shall undertake the necessary website and social media page building procedures,
provide link and public information in coordination with PMHA CDO, MIS. OR.

3. The SESUX shall provide PMHA CDO, MIS. OR the data on people who wants to be linked
with them and be provided with the “telekonsulta”.

4. The SESUX and PMHA CDO, MIS. OR shall undertake the promotion and dissemination of
information to generate support and participation among the PMHA CDO, MIS. OR
employees, families, and the public.

5. The PMHA CDO, MIS. OR shall provide assistance to the members of SESUX.

6. The SESUX and PMHA CDO, MIS. OR shall maintain a system of liaison thru designated
representative/s to maintain effective and efficient coordination in the implementation of this
MOA.
 This agreement shall take effect on the date of its execution, however, either of the Parties may
terminate this Agreement on the basis of breach or default by any of them by providing the defaulting or
breaching Party with a written notice within fifteen (15) days from such default or breach and the
defaulting or breaching Party fails to remedy such default or breach within five (5) days from receiving
the written notice of the same.

every page thereof.
