9:02 – 9:10 AM Introducing self and stating the purpose of home visit

9:11 – 9:16 AM Evaluating the household and immediate environment

9:17 – 9:37 AM Conducting interviews to the family regarding to their health


concerns and previous surgeries or diseases and how it was dealt
or health history

9:38 – 9:40 AM Addressing with “po” and “opo” and saying ma’am or sir not
calling them on their first names

9:41 – 9:56 AM Measures height and weight to each member of the family.

9:57 – 10:13 AM Checking of vital signs;’ Blood Pressure, Respiratory Rate, Pulse
Rate, Oxygen Saturation, and Temperature

10:13 – 10:28 AM Asking family members regarding their problems, issues,


concerns within their household and the community itself also
shows spot map where the current location of the family and
which is the nearest health care if immediate medical attention is
need.

Documenting the important data collected.

Informing the family for the next home visit.

MATERIALS:
The materials used during first home visit are vital sign measuring tools such as
sphygmomanometer, stethoscope, thermometer, wrist watch, and pulse oximeter. And tools to
measure body max index such as tape measure, and weighing scale. Face masks, clean gloves,
alcohol, notebook and black ballpen are also used in conducting the home visit.
EVALUATION: (Based on the objectives set in the Home Visit Plan)
After 3 hours of home visit the student nurse was able to collect data for assessment and
possible problems and attempts to establish rapport between the family. And was able to
assess physical, social, cultural, environmental and health status of the members of the family
together with the vital signs taking with the use of skills learned in assessing vital signs, assessed
and give appropriate health teachings regarding in their living condition concerning their health
practices, and was able to illustrate location of the family and where the family could go if
immediate medical attention is needed.
