I, EDGAR A. CUYA, of legal old, married, and resident of Villa

Corozan Subdivision, Del Rosario, Naga City, under oath, declare
that:

1. I am an employee of the Provincial Government of Camarines

Sur, assigned at General Services Office Capitol Compound,
Cadlan, Pili, Camarines Sur;

2. During my employment with the Provincial Government of

Camarines Sur, I was never charged of any civil,
administrative nor criminal case in any court, office or
agency, such as the Ombudsman;

3. During my employment with the Provincial Government of

Camarines Sur, I did not commit any misdemeanor or
wrongdoings and was never charge of any administrative nor
criminal case;

4. There is no administrative or criminal case filed against me

that is pending before any court, office or tribunal;

5. I am executing this affidavit to attest to the truth of the

foregoing and for whatever legal purpose this may serve.
