RETAINERSHIP AGREEMENT

KNOW ALL MEN BY THESE PRESENTS:

This AGREEMENT entered into by and between (name of client), Filipino, of legal age,
residing at _________________ (hereinafter referred to as the First Party);

- And -

_____________________________, a law firm organized under and by virtue of the laws of the
Republic of the Philippines as a general professional partnership, with principal office at 1252 Mindoro
St. Paco Manila, Philippines and represented in this act by Atty. Anthony Menzon (hereinafter referred to
as Second Party).

WITNESSETH:

1. That the First Party has retained the Second Party as Legal Counsel to represent the former in
the case for the adoption of Ivy Mallorca.

2. That the First Party has agreed to pay the Second Party the following fees, to wit:

2.1. An Acceptance Fee of One Hundred Thousand Pesos (Php 100,000);

2.2. An Appearance Fee of: Three Thousand Pesos (Php 3,000.00) for appearances within
Metro Manila; Five Thousand Pesos (Php 5,000.00) for appearances in areas near the
vicinity of Metro Manila; Seven Thousand Pesos (Php 7,000.00) for appearances in other
areas outside of Metro Manila.

2.3. All other incidental and miscellaneous expenses attendant to and necessary to the pursuit of
the First Party’s cause, including court/docket fees, transportation expenses, publication cost
and all other necessary expenses shall be reimbursed by the latter upon proper billing by the
Second Party.

3. That this Retainership Agreement shall not include pursuit of appeals in other tribunals.

4. That this Retainership Agreement observes the strictest confidentiality on whatever information that
may be involve during the attorney-client relationship.
