EXENT OF DAMAGE : Ring Finger (Left) pricked by a cross brace

DESCRIPTION OF ACCIDENT:

At the given date and time, in the course of dragging some good lumbers at the
fabrication area, Mr. Ariel P. Godoy accidentally got pricked on his left hand by a cross
brace.

RELATED FACTS:

1.) It was disclosed that Mr. Ariel Godoy at that time was tasked to haul
lumber that can be used for the table forming.
2.) Pulling the wood, his left swung sideway and accidentally hit a cross
brace.

ACTIONS TAKEN:

1.) Mr. Ariel P. Godoy was given first aid treatment at the Site Clinic.

RECOMMENDATION:

1.) To be referred to St. Patrick Clinic for the anti-tetanus injection.

2.) To be discussed in the next toolbox meeting.
