DATE/TIME OF ACCIDENT: August 15, 2008 / 8:50 A.M.
PLACE OF ACCIDENT : Ground Floor
EXTENT OF INJURY : Cut @ left middle finger

DESCRIPTION OF ACCIDENT:
On August 15, 2008, at around 8:50 A.M. Mr. Enrico C. Lerum was accidentally
hit by a steel louver causing a cut at his left middle finger.

RELATED FACTS:

1.) At the given date and time, while Enrico C. Lerum was loading a steel
louver into the mini dump truck, it slipped from his grip and accidentally
pinned his left hand .
2.) Mr. Enrico Lerum sustained laceration on his left middle finger.

RECOMMENDATION:

1.) To be referred to St. Patrick Clinic for further medication

2.) To be discussed in the next toolbox meeting.
