This is an open book written test. In the time allocated, you are to answer all of the following questions. Make sure you:

Use a pen. Assessments written in pencil will not be accepted.
Ask your assessor if you do not understand a question. Whist your assessor cannot tell you the answer, he/she may be able to re-
word the question for you
Do not talk to your classmates. If you are caught talking, you will be asked to leave and your assessment will not be marked.
Do not cheat or plagiarise the work of others. Anyone caught cheating will automatically be marked Not Yet Competent for this unit.
There are NO EXCEPTIONS to this rule.
QUESTIONS
1. For this task, you are to write a report about a time you have provided individualised
support to a client in the community services industry. If you have not provided
individualised support for a client in the community services industry, please complete
this in conjunction with your observation task. Your report must include details on the
following:
a. How you determined the support needs of the client, including:
i. How you clarified your own role in implementing the plan
To have a clarification of my own role in implementing the plan, I did the following to determine
the support needs of the client;
1. I read the care plan carefully and tried to understand the goals
2. I didn’t understand some of the plan and its goals, so I talked to my supervisor and the
nurse
3. I talked to the client by myself about the support needs and their preferences
4. I reviewed the notes written by the doctor
5. I reviewed all the notes when I was not clear and make myself clear about the care and
the plans needed to achieve the set goals
ii. Processes you followed to confirm the plan details with clients, and their
families and carers if appropriate
The Processes which I followed to confirm the plan details with clients, and their families and
carers are;
1. I talked and discussed the plans with the client’s family and friends
2. I asked the client about their choices and encourage them to provide the preferences
3. I maintained the privacy and confidentiality of the client
4. I talked to the manger and supervisor with the confirmation of plan details
iii. How you ensured that the client was aware their rights and of complaints
procedures
I ensured the client was aware of their rights and of complaints procedures by discussing and
talking with the clients and make sure that the client knows about his rights and the freedom to
have the choices and preferences.
For the preparation of support activities, including information regarding assembling and
required equipment, I made sure I can use the following supporting equipment;
1. Sphygmomanometer
2. Hoist
3. Wheelchair
4. Thermometer
5. Toileting chair
6. Glucometer
ii. How you developed and maintained trust with the client
iii. What methods you used to show respect to all involved throughout the
process
I used the following methods to show respect to all involved throughout the process;
1. I respect their culture, values and choices
2. I was honest and polite to all involved
3. I maintained the privacy and confidentiality
4. I offered the help anytime they needed
5. I always asked for the permission
6. I took the informed consent of every procedure
I monitored my work by taking feedbacks from the clients, my senior staff, client’s family and
questioned them whether they are happy with my care or not.

iii. Did the plan require reviewing? If so; why, who did you discuss it with and
how did that discussion support the client’s self-determination?
Yes, the plan requires reviewing. This is because the client’s need and choices may
change.
If the plan didn’t work then the plan must be changed according to the client’s needs
with the associated personnel for example; if the client doesn’t like the food then I
would talk to the dietician who can change the food menu as the client’s preferences
and dietary requirements which supports the client’s self-determination

2. How would you work with a person to identify actions and activities that support their
needs in relation to the individualised plan?
To identify the actions and activities that support the client’s needs in relation to the
individualized plan, I would work with a person by;
-I will ask their hobbies and activities
-By looking at care plan
-By discussing with client about their interests, likes, dislikes
-By discussing with client’s family
3. What would you do to ensure you provided support to a person according to the duty of
care and dignity of risk requirements?

To ensure that the provided support is according to the duty of care, I made sure of the
following;
1. Policies and rules of an organization are fulfilled
2. Both positive and negative feedback from client and other staff are taken
3. Privacy and confidentiality of the client are maintained
I did the following to ensure that the client’s dignity of risk requirements is provided;
1. I supervised the client all the time
2. I was present all the time to identify and avoid potential hazards
4. What assistance can you provide to ensure a person maintains a clean, comfortable,
safe and healthy environment?
To ensure a client is clean, safe and is in healthy environment, I can provide the following
assistance;
-Assisting the client with personal hygiene
-Identify any hazards and report them
-Cleaning the environment when necessary
8. What would you have to do to comply with your organisations reporting requirements?
3. If you can't eliminate the risk, at that point advise to the supervisor or the high
authority
5. Make a risk sign and put it on the hazard zone to prevent any accident
10. What should you do if you identify a client whose needs have not been met?
I would do the following if I identify a client’s needs have not been met;
-Tell the supervisor and consult with client on how the needs might be met
-Discuss with client and hence put changes into the care plan
13. What are two factors that affect people using your support services?
The two factors that affect people using the support services are;
-Financial Position
-Cultural and language barriers
14. What practices have you used to support skill maintenance and development?
I have used the following practices to support skill maintenance and development;
1. Cardiopulmonary resuscitation
2. Attend training when offered in the workplace
