PURCHASE OF RICE FOR COMMUNITY ASSISTANCE DURING COVID-19 PANDEMIC

KNOW ALL MEN BY THESE PRESENTS:

This CONTRACT AGREEMENT, made this _____ day of Ocotber 2020 by and between the
Barangay Government of Barangay Villaflor, Municipality of Loboc, Province of Bohol,
Philippines, represented by Hon. Charles M. Adorable, Barangay Captain of Barangay Villaflor,
Municipality of Loboc, Province of Bohol, (hereinafter called the ENTITY), as the PARTY OF THE
FIRST PART and CSL AGRIVET SUPPLIES, represented by Chito S. Luminarias, Proprietor, with
office address at M. Torralba Ext., Poblacion III, Tagbilaran City, Bohol Philippines (hereinafter
called the “CONTRACTOR”), as the PARTY OF THE SECOND PART,

WITNESSETH:

WHEREAS, the Entity is desirous that PURCHASE OF RICE FOR COMMUNITY ASSISTANCE
DURING COVID-19 PANDEMIC, VILLAFLOR, Loboc, Bohol, belonging to the Barangay
Government of barangay Villaflor (hereinafter called “the Project”) and the Entity has accepted
the bid for One Hundred Sixty Nine Thousand Four Hundred Sixty Pesos (Php 169, 460.00) by
the CONTRACTOR for the execution and completion of such Works and the remedying of any
defects therein.

NOW THIS CONTRACT AGREEMENT WITNESSETH AS FOLLOWS:

1. In this CONTRACT AGREEMENT, words and expressions shall have the same meanings
and are respectively assigned to them in the Conditions of Contract hereinafter referred
to.
2. The following documents shall be attached, deemed to form, and be read and construed
as part of this CONTRACT AGREEMENT, to wit:
(a) Contract Agreement;
(b) Specifications
(c) Invitation to Apply for Eligibility and to Bid
(d) Instructions to Bidders
(e) Bid Data Sheet
(f) Bid form, including all the documents/statements contained in the Bidder’s
bidding envelopes, as annexes;
(g) Eligibility requirements, documents and/or statements;
(h) Performance Security;
(i) Notice of Award of Contract and the Bidder’s conforme thereto;
(j) Other contract documents that may be required by existing laws and/or the
Entity

3. The Contractor shall complete the Works within a period of FIFTEEN (15) Calendar Days
from the effective date the Entity shall have notified the Contractor in writing to
commence the delivery, unless an additional time is granted in writing by the Entity.

4. In consideration of the payments to be made by the Entity to the Contractor as

hereinafter mentioned, the Contractor hereby covenants with the Entity to execute and
complete the deliveries and remedy any defects therein in conformity with the
provisions of this Contract Agreement in all respects.
 5. In consideration of the execution and completion of the Works by the Contractor, the
Entity hereby covenants to pay the Contractor the unit prices in the Contractor’s
Calculated Bid, as applied to the actual quantities accomplished as certified by the
Procuring Entity under the provisions of this Contract Agreement, at the times and in
the manner prescribed by this Contract Agreement.

IN WITNESS whereof, the parties thereto have caused this CONTRACT AGREEMENT to
be executed the day and year first before written.
