AFFIDAVIT

I, ELEANOR J. ARCIAGA, of legal age, Filipino, married, and presently

residing at B5 L28 Won St., Villa Carolina 1, Tunasan, Muntinlpa City, after having been
duly sworn to in accordance with law, do hereby depose and say: that-

1. I am the mother of CHRISTOPHER J. ARCIAGA who is a Registered

Nurse by profession;

2. My son CHRISTOPHER J. ARCIAGA was previously employed at

OSPITAL NG MUNTINLUPA designated as NURSE 1 from October 19,
2015 up to January 25, 2018 assigned at MEDICAL SURGICAL WARD
and designated as NURSE 1 from January 26, 2018-October 10, 2018
assigned at INTENSIVE CARE UNIT. Copy of Certification from the
said hospital is hereto attached as Annex “A”;

3. He was also previously employed at SOUTH CITY HOSPITAL AND

MEDICAL CENTER INC. designated as Nursing Supervisor assigned at
the Intensive Care Unit from October 19, 2018 up to November 1, 2019.
Copy of Certification from the said hostpital is hereto attached as Annex
“B”;

4. I execute this instrument in order to attest the truth of the above facts for
all legal intents and purposes this may serve.
