REFLECTIVE JOURNAL FOR MODULE # 3

Today is Thursday, February 4, 2021. We discussed about the fetal,


baby, and adult circulation as well with the complications in the
cardiovascular system. We also discussed about the subfertile couple, I
learned that infertility is not being able to get pregnant despite having
frequent, unprotected sex for at least a year for most couples. Infertility may
result from an issue with either you or your partner, or a combination of
factors that prevent pregnancy. Sterility is the inability to conceive because
maybe there is a condition that could affect a woman to be pregnant or
husband to ejaculate. Subfertility, the couple needs a health care provider, a
doctor, they need advice, they can conceive but less likely to do it without the
aid of medical provider. I learned also about the fetal circulation where there
are 5 adaptation and these were the umbilical vein, Ductus venosus which
allows the blood go to the inferior vena cava , the foramen ovale sending
blood from the right atrium to left atrium, then the ductus arteriosus allows
blood to flow from pulmonary artery to the aorta since the blood cannot go to
the lungs because of increase/high resistance and high pressure so the blood
will directly go to the aorta where there is low pressure and resistance, it’s like
in the fetal circulation the heart only serves as a bridge for the deoxygenated
blood to flow and become oxygenated. Last is the umbilical arterioles that
allows blood to flow from internal iliac artery, to umbilical artery and back to
the placenta. Into the placenta because the placenta has a very low
resistance so the blood can enter or divert towards it. Okay, after birth when
the placenta removed, when removing the placenta we need to clump the
cord but there is also a natural clumper and that is the wharton’s jelly, when
the tempature falls, it will squeeze the two umbilical arteriesfor contraction and
then the placenta will have a high resistance from blood, in few days when the
placenta got removed there will be no blood flowing in the umbilical veins,
there will be clogging and it become unused. So after birth the circulation will
go back to normal and the lungs will have low resistance and allow blood to
flow for oxygenation and there will be dilation and then the usual physiology of
oxygenation will follow, from vena cavas to arteries, to ventricles, to
pulmonary arteries, to lungs and veins and to the aorta until it will give
oxygenated blood to all systems of the body. I also learned about some
cardiovascular problems and one I could remember is the coartation of the
aorta means narrowing of the aorta and will pose complication like lowering
blood flow that would cause poor oxygenation of blood to flow all over the
systems of the body, there will be leg pain and pain on extremities, or else will
appear bluish tint, angioplasty is one of the intervention. The pre-ductal and
the post-ductal, maybe that’s all. Thankful for this day that there are
something that I learned despite complications in the internet.
