CASE: A patient underwent Mastectomy with following Diagnosis:


• IMPAIRED PHYSICAL MOBILITY INTERVENTIONS

1. Note situations such as surgery, fractures, amputation, tubing’s (chest)


2. Monitor vital signs
3. Assist/have client reposition self on a regular schedule.
4. Used side rails of bed.
5. Schedule activities with adequate rest periods during the day
6. Encourage participation in self-care; occupational, diversional, or recreational
activities
7. Encourage adequate intake of fluids and nutritious foods.
8. Encourage continuation of light exercises
9. Provide client with ample time to perform mobility-related task.
10. Administer medications prior to activity as needed for pain prescribed by
physician.

• IMPAIRED SKIN INTEGRITY INTERVENTIONS


1. Inspect the affected skin area, noting capillary blanching/refill, redness, and
swelling.

2. Keep the area clean/dry, carefully dress wounds, support incision.


3. Use appropriate barrier dressing, wound coverings, and skin-protective agents
4. Demonstrate appropriate positions for pressure relief.
5. Establish a turning or repositioning schedule
6. Keep bedclothes dry, lose and free of wrinkles, crumbs.
7. Discuss Importance of adequate nutrition (especially fluids, proteins, vitamins B
and C, iron and Calories).
8. Provide information about signs of wound infection and order complications to
report.
9. Encourage verbalization of feelings, perceptions and fears.
10. Assist client to learn stress reduction and alternate therapy techniques

• SITUATIONAL LOW SELF ESTEEM INTERVENTIONS


1. Establish rapport
2. Provide privacy
3. Act as a role model for the patient or significant others in healthy expression of feelings
or concerns.
4. Apply active listening and open-ended questions.
5. Help client to identify environmental factors which increase risk for low self-esteem.
6. Encourage creative problem solving through writing exercises.
7. Encourage client to verbalize thoughts and feelings about the current situation.
8. Help the client to identify the resources and social support network available to them at
this time.
9. Educate the female client about self-esteem differences between genders, and encourage
exploration.
10. Assess the client for symptoms of depression and anxiety. Refer to specialist as needed.
