Pursuant to your request we are submitting herewith my proposal to render

professional services for the company. The scope of our services shall include the
following:

1. Regular accounting services in order to ascertain compliance with BIR

and SEC regulations.

2. Preparation of monthly VAT returns, withholding tax returns on

compensation and expanded.(2550M & 2550Q)

3. Preparation of quarterly Income Tax Return.(1702Q)

4. Preparation and submission of summary of Sales & Purchases (Vat

Relief)

For the above services, we propose to bill you FOUR THOUSAND

PESOS (4,000.00) monthly payable.

For other services enumerated below, we shall bill you on a case to case
basis:

1. Represent the company with various government agencies such as BIR,

SEC and Local Government.

2. Yearly registration of books of accounts.

Cash Receipts
Disbursement Book
Journal
Ledger
 3. Renewal of mayor’s permit and municipal license.
(3,000 for retainer fee)

4. Submission to SEC of yearly reportorial requirements (GIS)

(2,000 for retainer fee)

5. Preparation of computation of SSS, Philhealth & Pag-ibig remittance.

(2,000 for retainer fee)

6. Preparation of year-end income tax return and financial statements in

compliance with requirements of BIR & SEC.
(5,000 retainer fee, Income tax Annual)
*not include CPA FEE for signature
