That I MELODY CAÑETE, of legal age, single, Filipino, and a

resident of Ormoc City, Leyte, Philippines, after having been duly
sworn in accordance with the law hereby depose and say that;

I am one of the staffs of ORMOC CITY LODGING located at

Carlos Tan St., Ormoc City, Leyte, Philippines, owned by Mr. Antonio
Domino;

That I have been assigned as Front Desk Officer of the

aforementioned lodging house and the same is managed and operated
by our manager, Julian Augustus Domino; that aside from myself and
Mr. Julian Augustus Domino, hereunder are the other staff of ORMOC
CITY LODGING with their respective destinations and/or functions;
