SWORN AFFIDAVIT

I, SHEENALYN VITANCUL OGNITA, of legal age Filipino and with residence address at 2-F
20th Avenue, Murphy, Brgy San Roque, Quezon City 1109, after having sworn to in accordance
with law do hereby deposes and state, that:

1. That this pertains to my unclaimed Special Amelioration Program (SAP) where disbursed
to my GCASH with assigned contact number: 0917332283 (Globe sim), it was so
happened that the aforementioned number was no longer existing being already
expired.

2. Hence, in order to collect my SAP claim, I respectfully request that my GCASH with
assigned old/previous Globe sim card number: 09171332283, be changed to my new
Globe sim card number: 09569865014.

3. That I am executing this affidavit to attest to the truth of the foregoing instrument and
for any legal purpose and intents this may serve.
