Equipment: Siemens 64-slice Dual Source SOMATOM DEFINITION CT machine.

Technique: Intravenous contrast enhanced helical [64 x 0.6 mm] transaxial triple phase
imaging of abdomen done on Siemens Somatom Definition 64-slice MDCT scanner.

OBSERVATIONS:

Liver is normal in size, outline and attenuation. No definite focal lesion is seen. Ano abnormal
focal enhancement in the arterial, venous of delayed phase images There is no evidence of any
dilatation of intrahepatic biliary radicles. The portal veins & hepatic veins appear normal.SMA
and SMV appears normal. Celiac artery and the branches appear normal. IMA appear normal.

Gall Bladder is well distended and shows normal wall thickness. No definite intraluminal
calculus is seen. No pericholecystic fluid or mass lesion is seen. CBD appears normal.

Approx 8 x 7.8 x 7.7 cm nearly well defined soft tissue density is observed at the level of
pancreatic head with no internal calcifications but internal hypodense degenerations. The
solid component of the lesion demonstrates arterial phase enhancement with persistent
enhancement in the venous and delayed phase images with no early wash off. The rest
pancreatic head is not separately identified from the mass. MPD is seen coursing posterior to
the lesion and appears prominent in the body and tail region measuring 3.3 mm. The body
and the tail of pancreas appear normal. The mass is seen to compress the portal confluence
and the adjacent SMV. However, the fat planes appear maintained. The mass is also seen to
displace the C-loop of the duodenum.

Spleen is normal in size, outline & attenuation. No focal lesion seen. Splenoportal axis is normal.

Both Kidneys are normal in size, outline, & attenuation. Pelvicalyceal systems of both the kidneys
appear normal. No focal lesion seen. No obvious calculi or calcifications seen.

Both suprarenal is normal.


The visualized small and large bowel loops are normal.
No free fluid is seen in the peritoneal cavity.
No lytic or sclerotic bony lesion is seen.

U Bladder is well distended with normal thickness and enhancement.

Uterus shows anterior wall 35 mm fibroid.


Both ovaries appear grossly normal.

CONCLUSION:
- Pancreatic soft tissue density heterogenous mass lesion with internal hypodensity
- s/o likely pancreatic adenoma (? Mucinous cystadenoma). Adv: CT guided
FNAC / BIOPSY after PTI / INR and consent.
- Uterine fibroid.
