Subject : Supply of Labor and Materials for Termite Control and General Pest Control (1 Lot)

Dear Fr. Paulino,

I hope that you receive this letter in good health, and in God’s Grace. Thank you very much for
considering us to submit our quotation for Supply of Labor and Materials for Termite and General
Pest Control requirements of our church located at the above-mentioned address. This is to
ensure uninterrupted and continuous protection from termite invasion of our church as well as
from other common household pests.

A. SCOPE OF SERVICES TO BE RENDERED: TERMITE CONTROL PROGRAM

1. SOIL POISONING
This is an application of about 5 li. of termiticidal solution per lineal meter to a trench or
drilled holes along the perimeter of the above-cited house area to create an unbroken
chemical barrier, making it not passable for termites to enter and destroy the wooden
structures and other cellulose-containing materials thru or either of the following:
1.1 Direct injection along the soil perimeters of the church of about 5 li termiticidal
solution per lineal meter using a modern pressurized soil rodder injector.
1.2 Drilling holes of about 0.5 – 0.75 m apart along the concrete perimeters of the
house using an electric roto-hammer drill, then inject about 1 gallon per hole of
termiticidal chemical solution using the same above equipment.
1.3 Drilled holes (3/8 in. diameter) after treatment shall be covered back with cement
or at your option will be marked for future follow-up treatment.

2. TROPHALAXIS DUSTING
This is an application of pyrethroid dust (anti-termite powder) to termite tunnels, crevices
and other wooden joints where there are signs of termite infestations

3. WOOD DRENCHING
Wooden structures (cabinets, ceiling joints, baseboard, jambs and others) will be
drenched with pyrethroid anti-termite formulations using pressurized sprayer. This is
both preventive and curative measures guaranteed to control termite and lengthen the
effective life strength of material.

4. SPRAYING
This is done in order to prevent/control termites and other structural insect pests. A
comprehensive spraying of all probable routes and habitation sites of these pests will be
conducted inside and immediate vicinity of the church/building.

5. TERMITE MOUND DEMOLITION

Termite mounds (punso) are homes of subterranean termites where the queen lives and
lays s eggs. All mounds in the target area will be excavated and treated. Excavation of
mounds shall be covered only within the client’s property area.

B. GENERAL PEST CONTROL PROGRAM (for the control of common household pests i.e.
cockroaches, ants and flies)
1. INDOOR RESIDUAL SPRAYING
This is comprehensive spraying in floor-wall junctions, baseboard and crack crevices of an
insecticide with long residual effect using compression sprayer to prevent/control population
build-up cockroaches, ants and other crawling insects.

2. SPACE TREATMENT / MISTING

This is an application of mist or fog for the killing of flying insects particularly flies and
mosquitoes. This will be applied using ULV or misting machine to cover outside area such
surrounding and garden area.

D. TECHNICAL SPECIFICATIONS OF CHEMICALS TO BE USED

Fipronil and / or other Pyrethroid termiticides = Termite Control (2% to 3% conc.). Beta-
Cyfluthrin and/or other Pyrethroid insecticides for household pests. The chemicals to be used
are FPA/FDA registered and approved for Termite Control and General Pest Control.

E. CONDITIONS
1. Frequency of service – one major comprehensive treatment and quarterly follow-up
monitoring/treatment within one (1) year for termite control. Whereas for General Pest
Control the frequency of service shall be quarterly treatment (every 3 months) to coincide
with termite monitoring treatment.
2. Payment shall be made upon completion of the major comprehensive termite control
treatment work or any date agreed by both party and for GPC shall be after each treatment.
3. Plumbing and electrical layout should be provided during termite comprehensive
treatment otherwise damaged on this part shall not be borne by us.
4. Guarantee shall be in effect for one (1) year and a quarterly monitoring within the
guarantee period. If there are any signs of termite infestations will be spot treated free. Any
excavation, renovation, landscaping or any similar activities that will displace/remove the
applied chemicals in the treated area should inform us for our appropriate termite control
recommendations otherwise guarantee shall not be in effect. Likewise, in portion or areas
where intervention will not be carried out as opt by the client which seems for us necessary
to be treated shall not be covered by this guarantee.
