MODULE OVERVIEW:

Nursing is a profession that requires intensive training on both theoretical and clinical perspective. This
module consists of NCM 104 and NCM 105 courses that deals with the principles and techniques of
caring for mothers during pregnancy, child birth, and postpartum periods, infants, children and family
and the application of principles and concepts on family and family health nursing process.

For this module, you will be providing nursing care to a mother that have problems or concerns with
obstetrics. Having gone through extensive theoretical inputs on the previous semester regarding care of
these clients, the next step is to apply these principles and techniques in nursing care. For this 100-hour
related learning experience, you will be catered to a flexible clinical learning experiences through this
self-instructional and learning module.

Remember, this might not be a substitute for the actual clinical experience, but this can help you
navigate through your patient care experiences in this alternative learning platform.

DESIRED LEARNING OUTCOMES:

At the end of this 100-hour related learning exposure, you are expected to:

1. Utilize the nursing process in the holistic care of client for the promotion and maintenance of health.

1.1. assess with the client his/her health condition and risk factors affecting health;

1.2. identify wellness /at risk nursing diagnosis;

1.3. plan with client appropriate interventions for health promotion and maintenance of health;
1.4. implement with client appropriate interventions for health promotion and health

maintenance taking into consideration relevant principles and techniques; and,

1.5. evaluate with client the progress of one’s health condition and outcomes of care.

2. Ensure a well-organized recording and reporting system.

3. Observe bioethical principles and the core values (love of God, caring, love country and of people).

4. Relate effectively with clients, members of the health team and others in work situations related to
nursing and health.

LEARNING ACTIVITIES:

Basic to every care that you provide to your patients, is the utilization of the nursing process. From the
time you receive the patient to your care up to the time your patients are ready for discharge no matter
what the health outcomes would be. You have always been doing assessment, planning of care,
providing nursing interventions and evaluating the care provided in the perspectives of the patient and
the significant others.

You are now on duty in the Delivery Room of the College of Maasin Hospital. In this 100-hour duty, you
are expected to take care of women on labor and delivery period. You are to take care of at least 1
patient per week. Your foci are the following:

 Vital sign monitoring

 Bedside care

 History taking and continuous patient assessment

 Administration of medication

 Intake and output monitoring (I&O)

 Special nursing procedure

 Planning of patient care

 Documentation of care

 Health teaching

 Discharge instructions

In your provision of the nursing care, you are reminded of the following behaviors:

 Professional behaviors- engage appropriate legal and ethical behaviors;

 Communication- communicate effectively with your patient, SO and health team;


 Clinical decision-making- develop critical thinking skills to make responsible decisions related to
assessing the patient;

 Caring interventions- maintaining a safe environment for the patient, including assessing social,
cultural and spiritual influences;

 Teaching and learning- provide health education to patient and SO;

 Collaboration and instructions- collaborate with patient in SO for follow-up care after discharge
As you start with this module, you are to consult and coordinate with your assigned clinical instructor.

CLINICAL SIMULATION:

Chief Complaint: Asthma in pregnancy

HPI: The patient is a 25-year-old woman with history of persistent asthma, who is now 10 weeks
pregnant. Asthma is currently well controlled; she is without new complaints, but is concerned
about her asthma treatment during pregnancy. Prior to her current asthma regimen, she had
frequent exacerbations.

Past Medical History: As above.

Review of Systems: As above.

Physical Exam: Unremarkable. Spirometry is normal.

Assessment and Plan: Asthma in pregnancy - continue current regimen and monitor spirometry.
Counseled patient regarding effects of asthma on pregnancy.

EXPLORATION AND APPLICATION:

To immerse yourself in the care management of your patient, you are expected to perform the
following:

1. Conduct history taking and physical assessment of your patient including the functional health
pattern.

2. Develop a Concept Map of your patient’s condition using the Concept Map Simulation Form.

3. Formulate a patient-centered care plan for your patient bases on his 3 prioritized problems. Integrate
in your care interventions significant bioethical and legal standards in patient care.

4. Make drug studies of all medications required by your patient in oral or parenteral form and
document it.

5. Create health teaching plan for your patient and his significant others.
6. Formulate discharge instructions for your patient.

7. In your 3-days duties per week, you are required to document your 2 days (Day 2 and 3). Be sure to
indicate vital signs and comprehensive data using your own clinical judgment.
