2.3.

13 Legal and Other Requirements


Works for Projects are covered by the legislative requirements of the Western
Australian Mine Safety and Inspection Legislation. These legislative requirements
impose a statutory obligation on all Employers, Employees, Contractors, Sub-
Contractors, Designers and Manufacturers.
It is the responsibility of each to ensure that the relevant statutory authority
legislative requirements are strictly adhered to as the minimum standard to apply on
this Project. MACA Interquip and Sub-Contractors are required to maintain copies of
the legislation on the Project and ensure that they are readily accessible at all times
by all of their employees and Sub Contractors.

2.3.17 HSE Objectives and Programs


MACA Interquip’s HSE Policy commits all Project activities to the objective of ‘Zero
Harm’ to people, property, the environment and the community.
All personnel shall work toward compliance with the Project HSE objective and take
appropriate actions to establish priorities, build team confidence, consult with the
workforce regularly, provide direction and create motivation towards action.
MACA Interquip shall define and set targets, applicable to the Contract, using the
following key principles:
 Work toward and promote ‘Zero Harm’ to people, the environment and the
community
 Embed targets in the HSE plan that embrace the MACA HSE Policy and this
HSE Management Plan
 Consistently achieve audit results in-excess of 80% compliance
 Schedule regular workplace inspections and audits
 Define actions which are specifically designed to achieve these targets
 Allocate responsibilities and sufficient resources to meet the defined actions.

I am of the view that we should obtain a copy of these audits for our records.

6.14.4 Certificates of Competency


All personnel operating mobile plant and other nominated equipment shall be required to hold an
appropriate operating high risk license or certificate/ letter of competency before they shall be permitted to
operate any plant or equipment on the project.
In addition, all such personnel may be individually assessed by a competent person appointed by their
employer, or as directed by the Project Manager, as competent to carry out their specific tasks in
accordance with such license or certificate of competency.
Where licenses and certificates are not issued for the operation of the particular plant standards of
competency shall be documented, and assessments shall be conducted by authorised, competent personnel
before operation of that plant shall be permitted on project.

Employees, including Sub Contractor employees, shall be required to present on request any certificates,
licences or tickets for any task or activity that requires such formal qualifications.
Challenge testing of all safety critical and designated tasks will be performed to ensure MACA Interquip
employees can competently perform their roles.

6.14.11 Health, Safety and Environment Statistical Reporting


Health, Safety and Environment statistical data shall be compiled and submitted into a project monthly
report in accordance with MACA Interquip requirements. All Health, Safety and Environment data shall
be uploaded into MACA Data and communicated to the Project Manager weekly and monthly as required
by MACA Interquip.

Project Office hours and manning shall be indicated separately from project site hours and manning. All
data supplied shall be recorded on the project MACA Interquip ‘Weekly Health, Safety and Environment
Report’ spreadsheet and the MACA Interquip database.

7.4 Job Safety Analysis (JSA) / SWMS


MACA Interquip shall maintain an up-to-date register of all JSA’s and associated
documentation using FO3139 - Job Safety Analysis and Permits Register.

7.7 Hazardous Substances


Hazardous substances are those which may cause harm to a person. Fit for purpose storage and controlled
use is critical to managing potential hazardous substance exposure.

Hazardous substances will only be used in accordance with the manufacturer’s relevant SDS which shall
be located near the substance.

All hazardous materials and chemicals will be accompanied by SDS.

Risk assessments must also be performed on hazardous substances to minimise exposure and protect the
health and wellbeing of employees in accordance with SOP5001 – Chemical Storage and Hazardous
Substances.

9. EMPLOYEE SELECTION, COMPETENCY, TRAINING AND BEHAVIOURS


9.1 Site Entry for Personnel

Personnel will be required to forward a copy of their driver’s licence, certificate(s) of competency and any
other qualifications to MACA Interquip at the interview stage and forwarded to site upon mobilisation. No
access to site will be granted without the appropriate completed induction and training.

9.6 Competency and Training


 All project personnel must hold current and relevant qualifications for the work they are
required to perform and these records shall be collected by HR at the interview stage / or
prior to signing relevant agreement, then maintained by the site responsible person for
training, able to be produced upon request;
 Assessments and documentation proving the competencies of employees are to be in
accordance with Nationally Accredited Standards;
 Personnel who operate equipment/plant must be certified, competent and approved to
operate each make and model of equipment / plant they operate, with their competencies
assessed, verified, documented, and recorded on the training matrix by their employer.

9.12 Training and Verification


Under no circumstance shall any person operate any mobile plant and equipment for which they have not
been trained. Employees authorised to operate mobile plant shall have been deemed competent to do so by
an approved and authorised person.

Operators of mobile plant shall carry their applicable vehicle licence with them at all times and be able to
produce their licence on request to verify licence currency and competency.

All light vehicle operators shall be in possession of a current Australian Drivers Licence. Operators who
have had their light vehicle licence suspended or removed are not permitted to operate any mobile plant.

It is MACA Interquip’s responsibility to ensure their operator licences and other accreditation is
maintained and updated in accordance to legislative and site requirements. MACA Interquip shall maintain
a documented training matrix that outlines the mobile plant and equipment competencies of each of its
employees.
