WAIVER AND RELEASE OF LIABILITY

In consideration of Pasig City making available the use of a bicycle under the bicycle sharing program, |

agree for myself, members of my family, heirs, executors, administrators, successors, and assigns that

the Pasig City Council, the City Environment & Natural Resources Office (CENRO), its employees,

officers, Barangay officials, and consultants shail not be liable for any damages arising from personal

injuries, including death, sustained by me as a result of the use of the bicycle. regardiess of whether

such injuries result, in whole or in part, from the negligence of Pasig City.

| am fully aware that bicycling involves the risk of serious injury or death and | accept and assume full

responsibility for any and all injuries, damages - both economic and non-economic, and losses of any

type which may occur to me as a result of my use of the bicycle made available by Pasig City. | hereby

fully and forever release and discharge Pasig City, its officials, City Council, employees, officers, and

consultants, from any and all claims, damages, or causes of action arising out of the use of the said

bicycle.

l expressly agree to indemnify and hold Pasig City harmless against any and all claims, damages, causes

of action, losses or judgements that may arise from my use of the said bicycle. In the event that a bicycle
is lost or stolen while it is rented under the access card registered to me, | may be liable Pesos: Twenty

Thousand (PHP20.000.00) for the City's loss of the bicycle.

| understand that Pasig City does not provide direct supervision, instruction, or assistance for the use of

the bicycles. | understand and agree that my use of the bicycles in the bicycle sharing program is only to

be undertaken on my own personal time, and is not within the course or scope of my employment. tf

agree to pay all damages to or loss of the bicycle unit caused by my negligent, reckless, or willful

actions.

| HAVE READ THE FOREGOING WAIVER AND RELEASE OF LIABILITY AND VOLUNTARILY
