Title of Activity
FIRST QUARTER REPORT CARDS OF SHS
STUDENTS.

1. To retrieve Learning modules/activity


sheets on Quarter 2, Week 2 which was
released to grade levels 7 to 12.
2. To properly distribute learner’s report
card to guardians/parents.
Objectives 3. To observe and exercise proper
implementation on retrieving of Learning
activity sheets and modules among
teachers and parents.
4. To observe and adhere to the protocol on
COVID-19 and other safety measures.

The event was prepared and rendered based


on the approved schedule of retrieval of LAS
Basis/ Reference
and distribution of First quarter report
cards.

The retrieval of learning activity sheets and


modules of quarter 2, week 2 were
successfully administered. There is 100%
retrieval of learning materials from SHS.
Only 20% of the learning materials from JHS
Evaluation Results were not returned.

The distribution of First quarter’s report


cards were successfully managed. There is
98% of released report cards .

Highlights of the activity 1. The retrieval of Learning materials were


properly administered by Matatalaib
High School Teachers headed by the
School Principal, Mr. Tito G. Ariola with
the support of non-teaching personnel.
2. Parents and guardians have observed to
strictly complied with the proper safety
measures in light of Covid-19 virus.
3. There is 100% retrieval of Learning
Materials in SHS.
4. There is 98% distributed learner’s Report
Cards on the SHS department.
 The School Head ensures safety
procedures during the retrieval of
Learning Materials and implement
continued adherence to safety
measures in preventing the spread of
Action Taken Covid-19 virus. Hence, proper social
distancing, wearing of face mask, face
shield and gloves were strictly
observed.
 SHS teachers communicated with
parents/guardians of the learner who
did not claim their report cards.
