AFFIDAVIT

I, CHRISTINE JOY TAPICAN, of legal age, Filipino and a resident of Brgy.

376 Aguilar St., Dampol II-B, Pulilan, Bulacan having been duly sworn to in
accordance with law depose and say that:

1. I am the owner of a business under the trade name CLAY’T JAPAG

registered in Department of Trade and Industry on 3 June 2012;

2. Since the period of registration of the aforementioned business, there

was no amount received to be recorded as either gross or net profit for
the reason that the business did not operate in any manner due to my
medical condition at that time;

3. Consequently, I registered a business in Department of Trade and

Industry under the trade name CJ Catering Services and Gen.
Merchandize in the year 2018 and since then, conducted its business
operation;

4. I am executing this affidavit to attest to the veracity of the foregoing

statements and for whatever other purpose this may legally serve.
