AMENDED EXTRA-JUDICIAL SETTLEMENT OF THE ESTATE

OF MERCEDES FAJARDO LEYNES

WITH WAIVER OF RIGHTS

KNOW ALL MEN BY THESEPRESENTS:

This Extra-Judicial Settlement of Mercedes Fajardo-Leynes is entered

into and executed this ____-- by and among the heirs:

WE MEDARDO T. LEYNES, MARK RYAN FAJARDO LEYNES and

ANGELO FAJARDO LEYNES, all of legal age all Filipinos, and LL
COMPULSORY HEIRS OF MERCEDES FAJARDO-LEYNES:

WITNESSETH :

1. That the late Mercedes Fajardo-Leynes acquired assets/benefits

during her lifetime and died intestate, a copy of her Death Certificate is
attached and made integral part hereto as Annex “A”.

2. That the late Mercedes Fajardo Leynes died on September 10, 2015
leaving as her sole heris her children and left no debts or obligation;

3. That the heir Medardo Leyenes hereby WAIVES all his rights and
share of inheritance in favor of his sons and herein co-heirs MARK RYAN
FAJARDO LEYNES and ANGELO FAJARDO LEYNES:

4. That the late Mercedes Fajardo-Leynes left intestate the following

assets/benefits to wit:

a. Burial/ and other benefits fro the Social Security System (SSS)
Government Service Insurance System (GSIS) and Pagi with the all
improvements existing thereon, consisting of 200 square meter-ibig Mutual
Fund and ;

b. Bank Deposit in the amount of _____________ from PNB Savings

Account with No. __________;

That we hereby, affirm that we have executed the foregoing instrument

out of our own and voluntary free will, without force or or intimidation or
violence upon our person, that we hereby warrant out title, ownership and
possession of the said asset/ benefits free from all liens and ecumbrances, with
the full right to enter into any agreement of whatever nature,,
