Reason for referral:


Jomar was recommended to attend an individual math intervention program to develop his basic numeracy
skills and promote independence in performing Math tasks. A structured and sequential method for learning was
recommended by his assessor.

Background Information:
Pertinent History: Attended reading remediation since Grade 1, Problem in word recognition and alphabet
symbols were diagnosed.

Status before remediation:


Jomar is a second grade pupil that was identified to be the recipient of the remedial plan for math because of his
characteristic when it comes to the math subject. He was identified to have the so-called dyslexia according to the
clinical psychologist. He was able to progress in many ways but still he needs to attend this remedial plan to concretize
his learnings in Math.
REMEDIAL PLAN
General Goal: This program aims to support the goals which were to promote JOMAR’S understanding on the concepts,
build his knowledge of efficient solving strategies and promote his Math language and word problem analysis skills.
Concepts will be reviewed using the methods introduced in this program and will be continually promoted through
regular practice drills.

AREA: Number Concept ( apply subtraction and multiplication of whole numbers up to 1000 including money in mathematical
problems and real-life situations.)
