AFFIDAVIT CERTIFYING DAMAGE

I, HODAEFAH ONTONGAN DICAMPONG, of legal age, 41 and resident of Barangay

Bubongamarawi, Marawi City, after having been sworn in accordance with law hereby depose and say:

1. That I am the owner of a house/store located at said Barangay

2. That on May 24, 2017. My family and I were forced to vacuate because of the rebellious act
committed by militants affiliated with Islamic State of Iraq and Syria (herein referred to as
“ISIS”) in the afternoon of May 23, 2017 in Marawi City that led to a war at such City;

3. That we temporarily stayed at Barangay BUADIDICO, TUGAYA LANAO DEL SUR as an

Internally Displace Person (IDP) as evidenced by Disaster Assistance and Family Access Card
(herein referred to as DAFACT) issued by Department of Social Welfare and Development
(DSWD), a copy of DAFAC is attached hereto Annex “A”;

4. That our Barangay, among others, were including in Cluster 3 kambalingan or in English term
homecoming, a third batch of barangays in Marawi City whose residents were allowed to come
back to their homes;

5. That last March 23, 2018, my family and I returned to our house/store in Marawi City;

6. That upon arriving at our home, my family and I was welcomed by our belongings scattered in
every part of our house some of which were damage as evidenced by photograph hereto as
“annex B”

7. That upon scrutiny, I found out that some of personal belongings were lost;

8. That upon inventory, our stolen property are as follows;

9. That in addition, the damage properties are as follows:

11. That I am executing this AFFIDAVIT to attest to the truth if the foregoing and to certify that I am
one of the Internally Displaced Person (IDP) whose property was stolen and damage due to the
Marawi Siege.
