I, DAVID C. BACTAD, of legal age and a resident of Barangay Nancayasan, Urdaneta

City, Pangasinan, would like to give my intent to be one of the Board of Directors of the
Urdaneta Memorial Park Incorporated again. I am very much aligned with the goals and
focus of our organization to support its ideals and plans and to reach its objectives.

At present, I am one of the original incorporator and also I have been the 1 st member of
the Board of Directors since our organization was established, and during those times
and I have experiences in joining meetings and as part of developing strategic solutions.
My administrative experience and proven leadership skills will prove useful to our
institution. I will offer a broad knowledge of programs, policies and implementation
strategies and looking forward to putting these towards a good cause.

I am confident that these qualities together with my commitment to work will be an asset
to our
