วิชำกำรประจ�ำปีครั้งที่ 30 ของสมำคมกำรศึกษำเรื่องควำมปวดแห่งประเทศไทย จึงมีเนื้อหำสอดคล้อง
กับกำรประชุมและมีควำมหลำกหลำยเกี่ยวข้องกับหลำยมิติของควำมปวด ซึ่งครอบคลุมกำรดูแลผู้ป่วย
ที่หลำกหลำย กลุ่มผู้ป่วยที่พบได้บ่อย และปัญหำเฉพำะเจำะจง

ควำมปวดเป็นอำกำรทีพ่ บบ่อยในเวชปฏิบตั ิ ท�ำให้ผปู้ ว่ ยต้องเผชิญกับควำมยำกล�ำบำกในกำรด�ำเนิน
ชีวติ โดยมีผลเสียต่อทัง้ ทำงร่ำงกำยและจิตใจของผูป้ ว่ ยและครอบครัว กำรรักษำอย่ำงถูกต้องจะส่งผลให้
คุณภำพชีวติ ของผูป้ ว่ ยและญำติดขี นึ้ ปัจจุบนั มีววิ ฒ
ั นำกำรกำรดูแลรักษำผูป้ ว่ ยทีม่ คี วำมปวดอย่ำงต่อเนือ่ ง
มีกำรศึกษำค้นคว้ำวิจยั ของกำรรักษำด้วยยำ หัตถกำรระงับปวด กำรผ่ำตัดและเทคโนโลยีทที่ นั สมัย โดย

หนังสือ “Pain Management in Vulnerable Patients” นี้ได้จัดท�ำขึ้นเป็นปีที่สอง ซึ่งมีเนื้อหำ
สอดคล้องกับปีแห่งกำรรณรงค์ตอ่ ต้ำนควำมปวดในผูป้ ว่ ยทีเ่ ปรำะบำง เช่น เด็ก คนชรำ สตรีตงั้ ครรภ์และ

โอกำสเสพติดยำแก้ปวดที่แพทย์ให้ได้บ่อย และผู้ป่วยติดยำเสพ ในผู้ป่วยกลุ่มนี้ ผู้ป่วยเสพติดกลุ่มนี้ ในระยะเริ่มต้นอำจไม่มี
ติดก็มีโอกำสเกิดภำวะปวดเรื้อรังได้มำกเช่นกัน เสี่ยงต่อกำรได้ พฤติกรรมแสวงหำยำจำกแหล่งทีผ่ ดิ กฎหมำย ผูป้ ว่ ยอำจจะไม่ได้
รับยำเกินขนำดท�ำให้เกิดภำวะแทรกซ้อนจนกระทั่งเสียชีวิตได้ มีปัญหำพฤติกรรมที่ผิดกฎหมำย เช่น ขโมยยำ เมำแล้วขับยำน

ควรงดให้นมบุตรระหว่ำงรับประทำนยำ โดยระหว่ำงหยุดให้นม ดังนั้นในปัจจุบัน paracetamol ยังคงเป็นยำที่แนะน�ำให้เลือก
บุตรควรบีบน�ำ้ นมทิง้ ด้วยเพือ่ กระตุน้ ให้มกี ำรผลิตน�ำ้ นมอย่ำงต่อ ใช้เป็นตัวเลือกแรกส�ำหรับบรรเทำปวดและลดไข้ในสตรีมคี รรภ์
เนื่อง และหลังจำกหยุดใช้ยำแล้วควรเว้นระยะห่ำงประมำณ โดยกำรใช้ยำนี้ใน therapeutic dose มีควำมปลอดภัยในทุก
4–5 เท่ำของค่ำครึ่งชีวิตของยำนั้นก่อนที่จะเริ่มให้นมแก่ทำรก ช่วงอำยุครรภ์ โดยแนะน�ำให้ใช้ระยะสัน้ ๆ ตำมควำมจ�ำเป็น11-13
อีกครั้ง สตรีให้นมบุตร
• ทำรกคลอดก่อนก�ำหนด ทำรกที่มีโรคหรือควำมผิด ส�ำหรับกำรใช้ในสตรีให้นมบุตร พบว่ำ paracetamol
ปกติของตับ ไต ระบบหัวใจ ระบบประสำท เป็นกลุ่มที่มีควำม สำมำรถผ่ำนสู่น�้ำนมได้โดยมีค่ำ MP ratio 0.76–1.00 และ
เสี่ยงสูงต่อกำรเกิดอำกำรไม่พึงประสงค์จำกยำที่อำจได้รับผ่ำน ค่ำ RID น้อยกว่ำร้อยละ 25,11 กำรใช้ยำในผู้ป่วยกลุ่มนี้มีควำม

ในสตรีมีครรภ์5 American Academy of Pediatrics (AAP) พบว่ำกำรใช้ NSAIDs มีควำมสัมพันธ์กบั กำรเกิดกำรแท้ง
จัดยำ paracetamol อยู่ในกลุ่มที่สำมำรถใช้ได้ในสตรีให้นม เอง (miscarriage หรือ spontaneous abortion) โดยควำม
บุตร (compatible with breastfeeding) เสี่ยงจะเพิ่มขึ้นเมื่อมีกำรเริ่มใช้ยำในช่วงที่คำดว่ำจะเกิดกำร
ปฏิสนธิ (conception) หรือเมื่อมีกำรใช้ยำเกินหนึ่งสัปดำห์13
Non-steroidal anti-inflammatory drugs (NSAIDs) ซึ่งผลนี้อำจเกิดเนื่องมำจำกฤทธิ์ของยำที่ยับยั้งกำรฝังตัวของ
สตรีมีครรภ์ blastocyst หรือท�ำให้กระบวนกำรฝังตัวผิดปกติ11,13 ดังนัน้ ควร
ข้อมูลในมนุษย์แสดงให้เห็นว่ำ ยำแก้ปวดในกลุม่ NSAIDs หลีกเลี่ยงกำรใช้ยำกลุ่มนี้ในสตรีที่ก�ำลังวำงแผนจะตั้งครรภ์11
อำจมีควำมเสี่ยงเมื่อใช้ในสตรีมีครรภ์ในช่วงไตรมำสที่ 1 และ ปัจจุบันยังไม่มีกำรศึกษำซึ่งแสดงผลที่ชัดเจนของกำร

โดยจำกรำยงำนของ The Global Burden of Disease Study อย่ำงไรก็ตำมปัจจุบนั มีกำรวิจยั เกีย่ วกับกลไกควำมปวดและกำร
2010 พบควำมชุกของกลุ่มอำกำรปวดจำกระบบกระดูกและ รักษำตำมกลไกต่ำง ๆ อย่ำงต่อเนือ่ งโดยเฉพำะกำรรักษำด้วยยำ
กล้ำมเนื้อในประชำกรมำกกว่ำ 2.9 พันล้ำนคน และเพิ่มขึ้นมำ ในปัจจุบันกำรรักษำผู้ป่วยกลุ่มโรคปวดข้อเรื้อรัง มีวิธี

บทน�า 2) โรคที่ไม่มีกำรอักเสบ (non-inflammatory joint
อำกำรปวดข้อเป็นอำกำรที่พบได้บ่อยในคนทั่วไป หำก pain)

อย่ำงไรก็ตำมผู้ป่วยกลุ่มนี้มักจะตอบสนองต่อกำรรักษำ
แบบอนุรักษ์ แต่ถ้ำอำกำรปวดไม่ดีขึ้นค่อยพิจำรณำกำรผ่ำตัด
สิ่งที่ต้องค�ำนึงถึงเพิ่มเติมในกรณีที่ต้องผ่ำตัด ได้แก่ ภำวะข้อต่อ

ที่กดทับเส้นประสำทออกร่วมกับท�ำกำรเชื่อมกระดูก16,17 ว่ำเส้นประสำทถูกผังผืดดึงรั้ง (Intradural fibrosis) ท�ำให้กำร
เรียงตัวของเสันประสำทในโพรงไขสันหลังผิดปกติ
4. Neuropathic pain, Epineural, Perineural,
and Intradural fibrosis (Arachnoiditis) ใช้ multidisciplinary pain clinic และ multidisciplinary
โรคกลุ่มนี้เป็นสำเหตุให้ผู้ป่วยหลังผ่ำตัดกระดูกสันหลัง approach ได้แก่ anticonvulsants (pregabalin, gabapentin),

ขึ้นรอบ ๆ โพรงไขสันหลังและดึงรั้งเส้นประสำท กำรวินิจฉัย จำกอำกำรปวดเรือ้ รัง20 กรณีทพี่ จิ ำรณำท�ำกำรผ่ำตัด แนะน�ำกำร
ได้จำกประวัติอำกำรของผู้ป่วยและส่งตรวจเพิ่มเติม ได้แก่ ผ่ำตัดแบบเปิดร่วมกับสลำยผังผืด (neurolysis) และป้องกันกำร
เอ็กซเรย์คอมพิวเตอร์ร่วมกับกำรฉีดสำรทึบรังสี หรือกำรถ่ำย เกิดผังผืดซ�้ำโดยใช้ free fat graft หรือ hyaluronic acid21,22
ภำพด้วยคลื่นแม่เหล็กไฟฟ้ำ18 (รูปที่ 4) นอกจำกนี้ปัจจุบันมี วำงตรงบริเวณที่มีผังผืดอยู่เดิม
กำรใช้ Epiduroscopy เพือ่ ช่วยในกำรวินจิ ฉัย พร้อมกับท�ำกำร
รักษำสลำยผังผืดไปพร้อมกันในรำยทีเ่ กิดผังผืดบริเวณ Epidural 5. Facet Joint Pain

อำกำรปวดภำยหลังผ่ำตัดกระดูกสันหลัง ที่เกิดจำกข้อ
กำรรักษำอำกำรปวดในผู้ป่วยกลุ่มนี้ ถ้ำมีผังผืดดึงรั้ง ต่ออักเสบพบได้ประมำณร้อยละ 5 มักจะเกิดจำกข้อต่อของ

การรักษาด้วยกายภาพบ�าบัด
จำกกำรทบทวน ไม่พบว่ำมีกำรศึกษำ level 1 อย่ำงไร
ก็ดีพบว่ำมีกำรศึกษำ level 2 ซึ่ง Timm และคณะ ได้เปรียบ
เทียบกำรรักษำโดยแบ่งผู้ป่วย 250 รำย เป็น 5 กลุ่มกำรรักษำ
รูปที่ 7 ผู้ป่วยอำยุ 84 ปี เคยผ่ำตัดกระดูกสันหลังเมื่อ 5 ปี ประกอบด้วย กำรให้ physical agent (hot pack, ultrasound,
