an ONCOLOGY SPECIALIST. She has been with us since January of 2014.

She withdraws a monthly salary of FORTY ONE THOUSAND PESOS (Php 41,000.00) plus
FIVE PERCENT (5%) per patient/case, and TEN THOUSAND PESOS (Php 10,000.00) for
meal and transportation allowances.

This certification is being issued upon the request of the above mentioned employee for
whatever purpose it may serve.
