AFFIDAVIT OF RELEASE AND QUITCLAIM WITH INDEMNITY UNDERTAKING

We, DIVINA TANKEKO VILLABROZA, widow; JERALD VINCENT TANKEKO

VILLABROZA, single; ACE LAURENCE TANKEKO VILLABROZA, single; DON ALPHONSE
TANKEKO VILLABROZA, single; and CHRISTOPHER JAMES TANKEKO VILLABROZA,
single; all of legal age, Filipinos, and residents of 10 Pajo Street, Barangay Quirino 2-A,
Quezon City, under oath hereby declare and state THAT:
1. We are the heirs of the deceased ALEXANDER MANAS VILLABROZA, who died
intestate on August 19, 2021 at 10 Pajo Street, Barangay Quirino 2-A, Quezon
City (the “Deceased”).

2. For and in consideration of our receipt of the amount of TWO HUNDRED

THOUSAND NINE HUNDRED NINETY-NINE PESOS AND NINETY-FOUR
CENTAVOS (₱ 200,999.94), representing proceeds of Savings Account no.
001490029042 of the Deceased with BDO Unibank Inc. Anonas-Aurora Branch
(the “Account”), we hereby expressly and absolutely remiss, release, and
forever discharge BDO Unibank Inc., its owners, directors, stockholders,
officers, employees, and assigns, (collectively, “BDO”) from any and all claims,
suits, actions, or causes of action which we, our successors/assigns now have,
or in the future claim to have against BDO in connection with the Account.

3. We hereby further obligate ourselves jointly, and severally to indemnify BDO

for any loss or damages it may sustain, in the event that any third person/s
shall succeed in establishing a right superior to that of ours concerning the
Account.
