Informed Consent Form

The Department of Psychology at Our Lady of Fatima University supports the practice of
protection of human participants in research.  The following will provide you with information
about the experiment that will help you in deciding whether or not you wish to participate.  If
you agree to participate, please be aware that you are free to withdraw at any point throughout
the duration of the experiment without any penalty.
In this study we will ask you to do a simple reaction time test
(https://humanbenchmark.com/tests/reactiontime). We will also ask you to play the game 3D aim
trainer (https://www.3daimtrainer.com). If you have sensitivity with flashing lights, please
inform the experimenter and the study will end now.  All information you provide will remain
confidential and will not be associated with your name.  If for any reason during this study you
do not feel comfortable, you may leave the laboratory and receive credit for the time you
participated and your information will be discarded.  Your participation in this study will require
approximately 30 minutes.  When this study is complete you will be provided with the results of
the experiment if you request them, and you will be free to ask any questions.  If you have any

Good day!
We are second year psychology students of Our Lady of Fatima University conducting an
experimental study with regards to the “Effect of Video Games to the Reaction time”. Our
respondents were chosen through the use of simple random sampling. We will ask you to
participate in an experiment wherein you will partake in an online reaction time test and play a
first-person shooter game in a span of 1 week.

Instructions:
1.) Visit the site (https://humanbenchmark.com/tests/reactiontime) and take the simple
reaction time test.
-When the red box turns green, click quickly as you can. This measures your reaction time
through milliseconds. You must test your reaction time before and after playing the aim
trainer.
2.) Screenshot the result of your first reaction time test for the day, send it to the researchers.

3.) Visit the site (https://www.3daimtrainer.com)
- Play the level tile frenzy in Hard difficulty. You must play this for five times per day for 1
week.
4.) After playing the aim trainer for five times, screenshot the results of every session and
send it to the researchers.

5.) Take the reaction time test (https://humanbenchmark.com/tests/reactiontime) again,
screenshot the results and send it to the researchers.

You may withdraw from this study for any reason. If you have sensitivity to lights or certain
colors, please do not hesitate to inform the researchers so we can immediately withdraw you
from our experiment. Any information and results that you share in this study would be handled
with the utmost confidentiality. Though the data that you shared may be published in a journal or
any format, rest assured that your identity will not be disclosed in the published material.
